2690715 Primary Facility 2018-01-26 Drilling/Alteration No Violations Noted Arrive on location: 12:30. Accompanied by DEP OGI, Ulyana Trynovich. Weather: Overcast, cold, ~20 degs. Cabot has completed setting new 20" Conductors, 16" mouse holes and Cellar Tech cellar units for (5) new wells on this pad, the Flower, T. 3H, 4H, 7 H, 8H and 10H wells. United Drilling has set all 20" conductors to 103' GL and 16" mouse holes to 25' GL. All were cemented to surface using Haliburton. (4) additional Cellar Tech cellar units have been set in place for wells to be permitted in future. All new wells 2" annular vent lines have been installed from cellars to poly vent tanks located at edge of location. Presently, well pad liner and drilling mats are being set for new drilling operations. - At opposite end of pad location, (2) existing wells, the Flowers, T. 1H and 2H were in production. See separate inspections for these producing wells. All was found in good condition, no violations were noted. Left location at 13:30.
2697701 Primary Facility 2018-02-15 Drilling/Alteration No Violations Noted Upon arrival, I met with Cabot's WSS, Sam Morgan. The PD #548 was RU over the Flowers, T. 007 well. Surface hole has been TD'd at ~881' and 13 3/8" casing was being ran in hole to 873'. Surface casing will be cemented this afternoon by Schlumberger.

- Rig has batch drilled the Surface intervals and has set and cemented Surface casings in the 004, 003, 010 and 008 wells on this pad. These wells Surface casings were also cemented to surface by Schlumberger. (See individual well insps for casing set depths and cementing info.) I reviewed Drillers logs and Surface casing cement reports for wells on location. All were found in good order. I also reviewed a CBL which Baker Hughes had just ran on the 004 wells Surface casing string. CBL looked good with apparent good bonding through out. I asked Mr. Morgan what sparked the CBL on this particular well. Mr. Morgan stated that Cabot has decided to run at least (1) CBL on a Surface string and (1) CBL on a Intermediate string, randomly on each pad to spot check the qu

2713331 Primary Facility 2018-03-27 Drilling/Alteration No Violations Noted Arrived onsite at: 14:30. Met with WSS, Marcus Cormier. PD Rig #548 was rigged up on the 8H well. Rig has TD'd the 8Hs Prod. hole at a depth of 12,874' and is presently circulating hole. Mr. Cormier plans to be running 5 1/2" Prod. casing in hole tomorrow morning and cementing same later tomorrow afternoon. Rig has previously completed drilling on the 4H, 3H & 10H. Rig has also completed the top hole interval, Surface and Intermediate, of the 7H well on pad. Rig will be skidding to resume drilling on the 7H well after setting and cementing Prod. casing in the 8H well. (See individual well insps for casing set depths and cementing info. for these wells.)

- There were also (2) United Drilling Conductor Rigs onsite setting 16¿ mouseholes and 20¿ conductor for the 6H & 9H wells. These conductors will be set at ~ 100¿ GL. United drilling has previously set 16¿ mousehole and 20¿ conductor for the 11H well. Spud notices were received for all (3) of these conductors. (1) other cellar has been set on pad for the 12H

2713437 Primary Facility 2018-03-22 Drilling/Alteration No Violations Noted Matt Shope, DEP O&G WQS and I arrived at the site at 11:23 to conduct an inspection. The weather conditions were partly cloudy skies, quite breezy and cold with temps in the upper 30's. Precision Drilling Rig # 548 is at the site. Mr. Shope and I went inside of the company man trailer in search of the company man shortly after arrival. He was not inside the trailer. According to the white board on the wall in the trailer, Precision is currently drilling the T Flower 008 well at depth of (2,805.20) feet. Shortly after reviewing the board and computer screen containing information relevant to drilling of the well and reviewing drill cuttings waste manifests, Mr. Shope and I proceeded to conduct an inspection of the site. We had observed that a portion of the secondary containment liner was not secured in (3) separate areas of the site. See photos. We eventually met Marcus Cormier, site company man, consultant of Cabot. We had a brief discussion regarding our site inspection near the rig and where additional we
2723324 Primary Facility 2018-04-20 Routine/Complete Inspection No Violations Noted I was accompanied by Ken Kennedy (OGI) and Casey Baldwin (WQS/ET). Precision Drilling Rig 546 is currently operating at the site. Prior to conducting the inspection, we met with Marcus Cormir (Company Man). The 12H well was being drilled at time of the inspection. 2756220 Primary Facility 2018-07-11 Routine/Complete Inspection No Violations Noted I conducted the inspection accompanied by Jon Sassi (EPS) and Casey Baldwin (WQS/ET). At the time of the inspection, the 11H Well plugs were being drilled out, a Coil Tubing Unit was on the 6H Well, and another service rig was being prepared for the 3H Well. 2798794 Primary Facility 2018-10-16 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected Casey Baldwin, DEP O&GWQS out of the Scranton District Office and I arrived at the site 11:46 in response to a reported spill of an unknown volume and time of a spill of hydraulic fluid to the pad surface. The weather conditions are mostly sunny skies, breezy and brisk, with temperatures in the mid 40's. On 10/16/18 at 8:56, Brody Webster, Environmental Health and Safety Supervisor of Cabot Oil and Gas contacted me to report the spill incident. He stated the following during our phone conversation. "A well tender discovered staining on the pad surface earlier this morning , 10/16/18, as a result of a spill of hydraulic fluid. A coil tube unit recently performed clean out operations on the T Flower 2H well. Hydraulic fluid was released out of the unit and impacted the pad surface. We are not able to calculate the actual volume of the fluid released at this time, but believe it is not more than (5) gallons, and consider this a small spill. GasSearch Drilling Services (GDS) is at the site in the process of
