2661634 Primary Facility 2017-11-14 Routine/Complete Inspection No Violations Noted On November 14, 2017 I conducted a routine E&S inspection of the Abbott well pad and tank farm. The site as a whole was in good shape with strong vegetation and a solid pad surface which was not muddy. There were no erosional issues apparent.

There were no violations noted.

2669119 Primary Facility 2017-12-08 Routine/Complete Inspection No Violations Noted I inspected the Abbott well site and tank farm on December 8, 2017. I performed a Routine Complete Inspection (RTNC) of the well labeling and erosion and sediment controls. The access road was solid and stable. The cut and fill slopes were well vegetated and the pad surface was solid. The vegetation on site was lush and full. The wells on site (5h and 11h) were properly labeled. Wells 115-22179, 115-22391 and 115-22390 were not drilled but are permitted. There were no erosional issues apparent at the time of inspection.

2757941 Primary Facility 2018-07-30 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man David Dewberry. United Drilling was on site to drill and set the conductor pipe on the 3H, 7H and 9H wells. The conductor pipe was set at 102' on all three wells. Halliburton arrived on site to cement the conductor pipe into place. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2763281 Primary Facility 2018-08-13 Routine/Complete Inspection No Violations Noted I performed a routine complete inspection of the Abbott well site. The well site was solid and stable despite recent monsoon like conditions. The slopes were stable and well vegetated. The access road was stable as well. There was extensive puddling however the recent rainfall has been extensive. The E & S log was up to date. The wells were properly labeled and there did not appear to be any erosional issues at the time of inspection.

There were no violations.

2806664 Primary Facility 2018-10-09 Drilling/Alteration No Violations Noted I arrived at the site at 13:31. Precision Drilling Rig #532 is at the location. I met with Jerry Dugas, Drilling Superintendent of Cabot Oil and Gas, and Aaron Wilhite, site company man, and completions consultant of Cabot. Precision Drilling Rig #532 is at the site in the process of drilling the D Abbott 3H well. The current drilling depth is (3,000) feet. Cabot submitted a Drilling Resumption After Break of 30 days or more notification to the Department on 10/7/18. The comments in the notification indicate Cabot estimated resuming drilling operations on the D Abbott 3H well with Precision Drilling Rig #532 at 12:00 PM on 10/8/18. The surface and intermediate casings have been cemented on the 3H well. The 7H well was the most recent well drilled to total depth. The vertical section of the 3H well is being drilled with water based drilling mud. The horizontal section of the well will be drilled with diesel based drilling mud, which is oil based mud mixed with diesel fuel. I departed from the site
2815930 Primary Facility 2018-12-13 Routine/Complete Inspection No Violations Noted I was originally onsite to perform an inspection in response to a release which occurred onsite. While onsite, I also performed a RTNC E&S inspection of the site.
    The weather was approximately 31 degrees F. and overcast.  The site signage was legible and up to date, as was the E&S log.
    The access road and pad surfaces were solid and stable, as was the cut and fill slopes of the tank and well pads.  The vegetation was visible through the snow and appeared healthy.  There were no erosional anomalies noted during the inspection. 


There were no violations related to E&S issues noted.

2849191 Primary Facility 2019-03-06 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The permit numbers on the entrance sign were correct. The well tag was on the well and correct. The cellar was empty and there was no gas detected. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 3/4/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
