Photo 2 shows the most recent E&S inspections conducted by COG contractor. As shown, there was a 9 day gap between the last weekly inspection and the last post rain event inspection. Please explain why this 9 day gap exists and what you will do to correct. The proposed topsoil stockpile next to the access road was not utilized. An under drain has been red lined and presumably installed on the west side of the combined pads. Other than minor red lining identifying the rip rap aprons for culverts 1 and 2, the site appears to have been built per plan dated 9/11/17. No violations noted other than the E&S inspection log discrepancy noted above. OK to spud. As photo's show, the site was snow covered which only allowed for a general inspection of the pad E&S controls. Unable to observe erosion control matting or possible hydro seeding.

2690895 Primary Facility 2018-01-31 Drilling/Alteration No Violations Noted Arrived on location at: 11:00. Weather: Clear, Cold, ~25 degs. Met with Cabot¿s WSS onsite, Larry Cheatwood. United Drilling was onsite setting Conductors and mouse holes for (6) New wells on Pad, The Pennay, G. 1H, 2H, 3H, 4H, 5H and 6H wells. Due to soft fill and some water encountered at ~80¿ - 90¿, while setting the 26¿ casing sleeves in the 1H and 3H wells, United Drilling will be setting 26¿ casing sleeves to 120¿ in all (6) wells, this rigs max depth. Cabot will then be moving in another H.A.D. Rig next week to drill, set and cement 20¿ Conductor casing to ~ 150¿ or consolidated rock. Cabot plans to be cementing 20¿ conductors to surface with Halliburton. All found in good order, no violations were noted at time of inspection. Left Location at: 12:00.
2702885 Primary Facility 2018-03-08 Routine/Complete Inspection No Violations Noted A routine inspection on Cabot's Pennay pad was conducted March 8th, 2018 to follow up on a Notice of Intent to Plug the Pennay 004 conductor section of the hole as a part of a Plug and Skid event after the operator reportedly had issues running 20" conductor pipe. Plugging work was scheduled for yesterday according to the Notice of Intent. At the time of inspection; no rig or Cabot personnel on location. The pad has seven wells at this time. Each of the seven wells on the pad have a visible 26" conductor, 20" conductor (except #004, 20" was separated downhole) and a mousehole only set. Mouseholes and conductors were capped with a plate, and it was unclear at the time of inspection if the 004 was plugged. I called the operator and was made aware the plugging contractor is on another pad and operations may begin this Saturday. There is a 20' section of 20" with a guide shoe where casing separated on bottom @ 140' - 161.5', however the operator believes they will pass through this section with tubing to plug b
2703230 Primary Facility 2018-02-26 Drilling/Alteration No Violations Noted Arrived on location at: 09:30. Weather: Clear, Cold, ~25 degs. Met with Cabot¿s WSS onsite, Larry Cheatwood. H.A.D. Rig was onsite setting Conductors for (6) New wells on Pad, The Pennay, G. 1H, 2H, 3H, 4H, 5H and 6H wells. Onsite in follow-up to phone call received from Cabot Drilling Engineer, Mike Stansfield on Friday 2/23/18. Mr. Stansfield was reporting the possible need to plug and skid the 4H well on this pad. In discussion with Mr. Cheatwood on location today, he explained that while setting the 20" conductor, at ~ 120' the conductor became stuck in hole at the bottom of previously set 26" casing. In attempts to free up 20" the bottom 20' section of 20" separated and was lost in hole. Prior to running the 20", 24" hole was drilled to - 220'. An attempt to fish out the 20" section of conductor will be made later today or tomorrow morning. Cabot will keep the Dept. updated as to the need for possible plug and skid of the 4H well.

2703983 Primary Facility 2018-03-08 Administrative/File Review No Violations Noted I received phone call from Cabot Drilling Engineer, Mike Stansfield on 2/27/18 informing that the fishing ops to retrieve the 20' section of 20" conductor were unsuccessful in the Pennay, G. 4H well. Cabot plans to be plugging and skidding the 4H well ~ 15' to east. This proposed skid will involve changing the top hole location coordinates for the 1H, 2H and 3H wells on this pad. I instructed Cabot to contact the permitting Dept. in Meadville to implement the prospective top hole coordinate changes. See attached:

I am entering this Plugging / Admin. Inspection to reflect the location changes for the 1H, 2H, 3H & 4H wells on this pad. The 5H and 6H wells will remain as originally proposed.

2707969 Primary Facility 2018-03-16 Routine/Complete Inspection No Violations Noted A routine inspection of the Pennay pad was conducted March 16, 2018. At the time of inspection, the were 7 wells total on the pad; 6 were active (conductors) and 1 was the recently plugged Pennay G 004 conductor. The plugged Pennay 004 original borehole was cut below ground level, capped with a plate, and marked with well name and API number. There were no obvious issues observed. The area around all 7 wells have been excavated, and prefabricated cellar tech containment cellars have been spotted in around each of the remaining 6 wells respectively. No obvious issues were observed. Attached are photos from the inspection and operator photos taken during plugging.
2794959 Primary Facility 2018-10-10 Incident- Response to Accident or Event No Violations Noted Frank Hoehle, DEP O&G WQS and I arrived at the site 9:58 in response to a spill of an estimated (30) gallons of recycled fluid to the pad surface. The weather conditions are partly sunny skies, mild with a light breeze, and temperatures in the 60's. Keane is the Cabot subcontractor in the process of conducting hydraulic fracturing (stimulation) operations. Larry Fulmer, Frac Superintendent, and consultant of Cabot Oil and Gas, and Steve Catalfamo, Professional Geologist , and Senior Environmental Health Specialist of Cabot, were at the site during the inspection. Mr. Catalfamo contacted me at 16:22 on 10/9/18 to report the spill incident. He provided me with the following details in reference to the spill. "As a result of over-pressurization on a Susquehanna Gas Field Services water truck, fluids were expelled out of a pressure relief valve on top of the truck. The spill occurred at 3:30 PM today, 10/9/18. The truck was preparing to offload the fluid into a frac tank in a tank farm on the tank pad se
