2703808 Primary Facility 2018-03-06 Preoperation Inspection No Violations Noted Today's inspection was conducted as a pre-operational inspection. The site has been constructed as per plan with minor red lined additions. No violations noted. OK to spud.
2739084 Primary Facility 2018-04-12 Drilling/Alteration No Violations Noted I arrived on the well pad section of the site at 11:20. The weather conditions are is cloudy skies with rain falling, increasing winds and temperatures in the mid to upper 40's. Precision Drilling Rig # 181 Super Single is on the location. Steve Catalfamo, Environmental Health and Safety Specialist, Eric Peterson, Drilling Environmental Healthy and Safety Consultant, Tony Ketterling, site company man, consultant and Daniel Copper all of Cabot Oil and Gas were on-site during the inspection. The current Drilling depth is (837) feet. Cabot personnel stated the following in reference to site activities. "This rig is a tophole rig currently drilling the Myers 5H well. We will drill this well to (2,050-2,100) feet and then cement the intermediate casing. The surface casing was cemented at a depth of (827) feet last night, 4-11-18. Precision Drilling Rig # 548 will follow this rig. We are drilling with water based mud. The 3H well will be drilled next to the intermediate section with the casing cemented." I departe
2763924 Primary Facility 2018-08-15 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Recent/current localized flooding due to heavy localized rain events are the primary reason the Department conducted todays inspection at this site. No erosional issues were identified. The design and implementation of E&S controls appear to have been quite effective. No violations noted.
2785732 Primary Facility 2018-09-25 Routine/Complete Inspection No Violations Noted Brody Webster contacted Jon Sassi at 15:22 hours on 9/24/18 to report a spill of (1) barrel, (42) gallons of synthetic oil based drilling mud (SOBM) to the Cabot Oil and Gas R Myers Well Pad surface. Mr. Webster provided the following details in reference to the spill. “The spill occurred on 9/24/18 between 14:24 and 14:30 hours. The spill occurred as a result of a pressure relief valve popping off on a tractor trailer transport truck while the truck was in the process of transferring SOBM from the Myers Pad to another Well Pad to be used during drilling of wells. Drilling of the wells on the Myers Pad has recently concluded and the rig has moved off the location. The (SOBM) was stored in frac tanks. The fluid leaked out of the top of the truck to the pad surface. Dark staining and standing water black in color was observed on the pad surface. A water truck removed the standing water. The synthetic oil based mud is not mixed with diesel fuel. Woodard and Curran will delineate the extent of the contamination
2828063 Primary Facility 2019-01-14 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection at the Myers R well pad. The well pad was quiet, with what appeared to be some clean up activity from the frack operation at the time of inspection. The weather was about 25 degrees F and it was sunny.

The entrance was well graded, relatively fresh stone, and had no fines pulled onto the public road.

The access roads to the two pads were both solid and stable, and the signs at the entrance were legible and the E&S log was up to date.

The well pad surface was well graded fairly recently it appeared. The tank pad surface was well graded also, with a solid, well graveled surface.

The cut and fill slopes for both pads appeared solid and stable. The vegetation left over from last growing season appeared adequate for soil retention.

The well signs were legible.

There were no violations noted at the time of inspection.
