2703795 Primary Facility 2018-03-06 Preoperation Inspection No Violations Noted Today's inspection was conducted as a pre-operational inspection. The site has been constructed as per plan with minor red lined additions. No violations noted. OK to spud.
2709095 Primary Facility 2018-03-21 Drilling/Alteration No Violations Noted Arrived onsite at: 10:30. Weather: Overcast, cold ~ 28 degs. Upon arrival I met with Cabot WSS, Larry Cheatwood. United Drilling had rigs onsite setting 20" conductors for (7) new wells on pad. The Meyers, R. 1H, 2H, 3H, 4H, 5H, 6H, 8H. A Spud notification was also received for the 10H well but, Mr. Cheatwood informed that this well will not be drilled due to a change in Cabot's plans. I requested that Cabot update the previously entered Spud Notification to reflect this change and avoid future confusion.

- All (7) wells conductors have been set at ~ 102' GL. Halliburton will be cementing these conductors later this afternoon. United Drilling was presently drilling for setting of (2) 16" mouse-holes for (2) additional proposed wells on pad, the 7H & 9H. These mouse-holes will be set at ~ 30' and cemented to surface. The remaining (7) 16" mouse-holes will be set and cemented tomorrow afternoon. The Cellar Tech cellar units will be set for all (9) wells next week. - I received a phone call from Cabots Drilling

2723096 Primary Facility 2018-04-18 Drilling/Alteration No Violations Noted Arrived onsite at: 10:00. Met with Cabot Co. Men, Glen Broussard and Aaron Wilhite. PD# 181 SL Rig was RU on the 3H well. Rig has just completed the 3H's Surface Casing "SC" cement job. The 3H SC was set at 825'. The cement job went well with ~ 66 bbls of cement returns to surface, floats held.

- Since my last inspection, Rig has set and cemented the the Surface and Intermediate casings in the 5H well. Both cement jobs went well with good returns to surface, floats held. (See individual 5H inspection for further details). - I reviewed cement job logs for the 3H and 5H wells, all were found in good order. - No violations were noted at time of inspection. Left site at 11:00.

2726604 Primary Facility 2018-05-01 Drilling/Alteration No Violations Noted Arrived on location at 11:00. Met with Cabot Co. Man Aaron Wilhite. PD #181 SL Rig is RU on the Myers, R. 2H well. Rig is presently down performing repairs to mud pumps. Aaron plans to be completed with pump repairs and be back to drilling later this afternoon. Rig is presently drilling the 2H's Intermediate interval at a depth of 1052' to a projected TD of ~ 2075'.

- Since my last inspection Rig has drilled, set and cemented surface and intermediate casings in the 3H and 1H wells top hole intervals. (See individual inspections for these wells for casing and cementing details). - I reviewed drillers logs, cement reports and casing tests performed for the well intervals drilled since my last inspection. All were found in good order. Rig and location were found in good overall condition. No concerns or violations were noted at time of inspection. Left site at 12:30.

2739066 Primary Facility 2018-04-12 Drilling/Alteration No Violations Noted I arrived on the well pad section of the site at 11:20. The weather conditions are is cloudy skies with rain falling, increasing winds and temperatures in the mid to upper 40's. Precision Drilling Rig # 181 Super Single is on the location. Steve Catalfamo, Environmental Health and Safety Specialist, Eric Peterson, Drilling Environmental Healthy and Safety Consultant, Tony Ketterling, site company man, consultant and Daniel Copper all of Cabot Oil and Gas were on-site during the inspection. The current Drilling depth is (837) feet. Cabot personnel stated the following in reference to site activities. "This rig is a tophole rig currently drilling the Myers 5H well. We will drill this well to (2,050-2,100) feet and then cement the intermediate casing. The surface casing was cemented at a depth of (827) feet last night, 4-11-18. Precision Drilling Rig # 548 will follow this rig. We are drilling with water based mud. The 3H well will be drilled next to the intermediate section with the casing cemented." I departe
2763905 Primary Facility 2018-08-15 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Recent/current localized flooding due to heavy localized rain events are the primary reason the Department conducted todays inspection at this site. No erosional issues were identified. The design and implementation of E&S controls appear to have been quite effective. No violations noted.
2785710 Primary Facility 2018-09-25 Routine/Complete Inspection No Violations Noted Brody Webster contacted Jon Sassi at 15:22 hours on 9/24/18 to report a spill of (1) barrel, (42) gallons of synthetic oil based drilling mud (SOBM) to the Cabot Oil and Gas R Myers Well Pad surface. Mr. Webster provided the following details in reference to the spill. “The spill occurred on 9/24/18 between 14:24 and 14:30 hours. The spill occurred as a result of a pressure relief valve popping off on a tractor trailer transport truck while the truck was in the process of transferring SOBM from the Myers Pad to another Well Pad to be used during drilling of wells. Drilling of the wells on the Myers Pad has recently concluded and the rig has moved off the location. The (SOBM) was stored in frac tanks. The fluid leaked out of the top of the truck to the pad surface. Dark staining and standing water black in color was observed on the pad surface. A water truck removed the standing water. The synthetic oil based mud is not mixed with diesel fuel. Woodard and Curran will delineate the extent of the contamination
2828076 Primary Facility 2019-01-14 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection at the Myers R well pad. The well pad was quiet, with what appeared to be some clean up activity from the frack operation at the time of inspection. The weather was about 25 degrees F and it was sunny.

The entrance was well graded, relatively fresh stone, and had no fines pulled onto the public road.

The access roads to the two pads were both solid and stable, and the signs at the entrance were legible and the E&S log was up to date.

The well pad surface was well graded fairly recently it appeared. The tank pad surface was well graded also, with a solid, well graveled surface.

The cut and fill slopes for both pads appeared solid and stable. The vegetation left over from last growing season appeared adequate for soil retention.

The well signs were legible.

There were no violations noted at the time of inspection.
