2621150 Primary Facility 2017-07-31 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 11:35a.m. for a new spud inspection.

I met with Brett Wambold of Selman & Associates. United Drilling was on site to drill the holes and run the conductor pipe on the 1H, 2H, 3H, 4H, 6H, and 8H wells. The 3H, 4H and 8H were drilled to TD at 94'. There was a rig on the 1H and the 6H, the rig on the 1H was running 26 pipe to 25' and the rig on the 6H was at TD of 94' and started pulling drill pipe. The 2H was at a depth of 70'. All the conductor pipe will be run to 94' and cemented in. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:20p.m.

2626660 Primary Facility 2017-08-17 Preoperation Inspection No Violations Noted I performed a preoperational inspection for the Tsourous A well pad. The well pad had all perimeter E and S controls in place and was well under construction at the time of inspection. The site had received torrential rain the previous day but no BMPs were adversely affected. The site was stable and there was some preliminary growth on sections that have already been completed. The contractor, GDS, was actively working on the pad surface at the time of inspection. All topsoil piles were stable and mulched and/or seeded if not being actively worked on. The RCE was stable and solid and was not choked up with mud and debris. No sediment appeared to be leaving the site.

I therefore recommend approval of this preoperational inspection.. There were no violations noted at the time of inspection.

2631050 Primary Facility 2017-08-31 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Perk McDowell. The surface and intermediate casings were cemented by Schlumberger with CemCrete cement. I took a walk around the location and there were no obvious concerns noticed.

2700617 Primary Facility 2018-02-27 Routine/Complete Inspection No Violations Noted At time of inspection.

I met with company men Steve Barrett and Ken Wilson. Keane was on site to frac the 1H, 2H, 3H, 4H, 6H, and 8H wells. They were just starting stage 22 out of 65 stages on the 2H when I arrived. The 1H and 3H were both completed with 48 stages. There were 21 stages completed out of 69 stages on the 4H. There were 22 stages completed out of 67 stages on the 6H. The 8H wasn't started yet but will have 32 stages when completed. I took a walk around the location there were no obvious concerns noticed. There were no violations noted.

2719962 Primary Facility 2018-04-19 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Tsourous well site on April 13, 2018 (Photo 1). The site had its last E&S inspection on April 13,2018 (Photo 16). The site had sediment being tracked from the site as well as running from the access road onto Lutz road. Lutz Road and Youngs Road were very muddy from truck traffic to the site and had a considerable amount of sediment pushed off the side (Photos 2, 18). This was in contrast to Lutz road beyond the access road to the well pad where the sediment was not nearly such a preponderance of sediment (Photo 17). The access road leading to the site was beset with problems. The entrance showed evidence of sediment leaving the site (Photo 2). The first culvert up the access road was occluded with sediment on both ends (Photo 4 and 5). There was also an unprotected topsoil pile near the entrance area (Photo 3). The slopes of the access road had sediment running off and accumulating around and beyond the filter sock BMPs (Photos 6,7,8,9,10). The sediment ran arou
2733237 Primary Facility 2018-05-21 Follow-up Inspection No Violations Noted I performed a follow up inspection of the Tsourous well site. The wells associated with this site are as follows: 115-22297, 115-22298, 115-22286, 115-22288, 115-22299, 115-22287. The areas of concern noted in the previous inspection have been addressed and repaired. There did not appear to be any additional erosional issues at the time of inspection. There were no violations noted.
2738773 Primary Facility 2018-06-06 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The well tag was on the well and correct. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. Fluid is going in a 500bbl flow back tank that is on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2765248 Primary Facility 2018-08-02 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:10. The weather conditions are cloudy skies, and breezy with light rain falling, with temperatures in the low to mid 70's as I arrived. Shortly after I arrived, light rain evolved into a moderate, steady rainfall. There was no activity or personnel at the site. All permitted wells listed on the Site ID sign are in production. I observed trash and debris scattered in some areas of the well pad and tank pad surface. The trash was most likely deposited on the ground surface of the site during completions operations, which have concluded in a relatively recent time frame. I departed from the site at 12:53.
2809101 Primary Facility 2018-11-29 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Tsourous well site. The well site is extensive and has a separate tank pad. The access road is quite long and steep but was plowed. The last E&S inspection was November 27, 2018.
    The weather was overcast and there was light snowfall the previous evening.  The  temperature was about 34 degrees.
    The road (Lutz Road) leading to the site was snowy and slushy.  There didn't appear to be any runoff along the roadside.
     The tank pad surface was snow covered and stable.  The infiltration basin, fill slope and topsoil pile all had good vegetation visible through the snow on them.  There didn't appear to be any erosional problems at the tank pad area.
    The well pad was expansive but had good growth on all slopes, visible through the snowcover.  The pad surface was mostly hard and partially plowed, with a few slushy areas.  The infiltration basin was mostly empty and appeared stable and growth was visible around it.  
    The wells were properly labeled and the
2835868 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was empty and there was no gas detected. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
