2641441 Primary Facility 2017-09-19 Routine/Complete Inspection No Violations Noted Inspection conducted 9/19/17. Today's inspection was conducted as a routine inspection. Good growth observed on both the old pad and the upgraded well site to include the tank pad. Improvements requested from the last inspection looked good. Photo 5 shows a fork blade cut in the overpack of the venting configuration for the 27H. As the photo shows, the produced fluid is right at the apparent cut. Steve Watson, COG EH&S Specialist was contacted about this, and stated that he would have the overpack replaced immediately. No violations were noted during this inspection.
2655664 Primary Facility 2017-10-24 Routine/Complete Inspection No Violations Noted Arrived at the site at 12:28. The weather conditions were partly cloudy skies, warm and breezy with temperatures in the 70's. Precision Drilling Rig #548 is on the location currently drilling the 31H well at a depth of (2,236.44). I met with Joel Franks and James Cheek, both site company men and consultants of Cabot Oil and Gas. Mr. Franks informed me of the following as far as the progress of site activities. "The intermediate casing was cemented at (2,124) feet and completed last night. The surface casing was cemented at (776) feet. This well will be drilled to total depth at (17,200) feet. We are currently drilling with water based drilling mud. The 16H-19H wells were in production but are currently shut in. The 22H-27H have been drilled to total depth. We still have to drill the 28H, 29H and 33H to total depth." I departed from the site at 14:03.
2671353 Primary Facility 2017-12-06 Drilling/Alteration No Violations Noted Arrived onsite at: 13:00. Weather: Clear, cool ~ 40 degs. Met with Co. Man Marcus Cormier. Precision #548 is presently running prod. casing in the the Teddick, M. 33H well. Rig has Td'd the 8 1/2" prod hole at a depth of 17,280' and plans to be setting prod. csg to a depth of 17, 250'. Prod. csg. will be cemented this afternoon using Halliburton. -This rig has recently Td'd (8) wells on pad, the Teddick, M. 22H, 23H, 24H, 25H, 26H, 27H, 28H and 31H. Rig will be drilling (1) additional well on this pad to Td, the Teddick, M. 29H. -The (4) exiting wells on this pad, the Teddick, M. 16H, 17H, 18H and 19H wells were shut in, in preparation for drilling the new wells on pad. (See individual well insps. for casing info.)

- The 22H well was previously TD'd on 8/23/17 to a depth of 15,849'. - Overall, Rig and location were found in good order at time of insp. No violations were noted. Left location at 14:30.

2719765 Primary Facility 2018-04-11 Routine/Complete Inspection No Violations Noted Today’s inspection was conducted as a routine inspection. Also present during the inspection was Jon Sassi, Department EPS. The Department requests a copy of the E&S inspections conducted by Cabot since 3/1/18 to present as documented in Photo # 1, a picture of the on-site E&S Inspection Log. Photo # 2 shows that the mud build up has been removed from Infiltration Berm # 2. Photo # 3 shows the entrance of the cross culvert which discharged all the mud to Infiltration Berm # 2 has been cleaned and stabilized. Photo # 7 shows show the location where sediment has been deposited in the farm field down gradient from the tank pad. This deposit is approximately 300’ off the LOD. Photo # 8 shows the sediment deposited in the farm field ag swale down gradient of Photo # 7. Photo # 9 shows the plume of deposited material half way between the ag field swale, Photo # 7 and Compost Filter Sock Sediment Trap # 2. 2732561 Primary Facility 2018-05-09 Routine/Complete Inspection No Violations Noted Today's inspection was conducted in response to Cabot's self reporting of a small spill at their Teddick Well Site. Also present during the inspection was Casey Baldwin, Department WQS and Steve Watson, Cabot Environmental Specialist. At 9:12 AM, Steve Watson contacted Jon Sassi, Department EPS to report that at earlier in the day, a water truck spilled production fluid on the lease road and 2 adjacent township roads, Beaman Hill Road and Waterford Road while hauling off production fluid from this site to Comteck for processing. Mr. Watson explained that while the truck was leaving, an emergency overflow pipe valving failed and allowed fluid to escape while the vehicle drove down steep grades. The estimated release was reported as 10 gallons which, based on conditions during the inspection and photos provided by Mr. Watson, the Department finds to be a reasonable estimate. All locations impacted were remediated with collected material currently being stored at the well pad in a lined roll off container.
