2479970 Primary Facility 2016-05-02 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:25 a.m. for a new spud inspection, United Drilling was on site drilling the mouse holes for the 5H and 11H, I met with drillers Tim Mullins and Doug Lutz, they said the conductor pipes for both wells were run and cemented 4/30/16 at 104'. I took a walk around the location and everything looked OK.

I left the site at 9:50 a.m.

2490146 Primary Facility 2016-06-03 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 10:15 a.m., Precision Rig 548 was on site and drilling the hole for the intermediate casing. I met with company man Perk McDowell, we took a walk around the location and everything looked OK.

Depth at time of inspection was 1723' I left the site at 11:20 a.m.

2491162 Primary Facility 2016-06-07 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 10:15 a.m., Precision Rig 548 was on site and drilling the hole for the production casing. I met with company man Perk McDowell, we took a walk around the location and everything looked OK.

Depth at time of inspection was 7635' Projected TD is 11591' I left the site at 11:40 a.m.

2539758 Primary Facility 2016-10-18 Routine/Complete Inspection No Violations Noted I arrived at the site at 13:20. Pipeline construction was in progress across the road from the tank pad. Hydraulic fracturing operations have apparently just have ceased on the well pad as crews were in the process of rigging down from the frac. There was no activity on the tank pad. An above ground storage tank, also known as a Lake Tank, is staged on the tank pad. This tank most likely contains freshwater that is used during stimulation of the wells. A tank farm containing frac tanks most likely storing recycled water used during stimulation of the wells is also staged on the pad. Recycled water is a mixture of flowback and production fluid generated from Cabot natural gas wells. Puddles of water from recent rainfall events have accumulated on and off containment on the tank pad. I used a Hanna Soil Test and Electrical Conductivity and & Temperature meter to record conductivity readings of damp soil near the tank farm containment berm and water in the tank farm containment in the Southwest Corner
2541656 Primary Facility 2016-12-02 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:55a.m., the well was recently completed and is in production. The well tag was on the well and correct. The cellar was about three quarters full of water with no bubbling noticed, the (20x13) vent pipe was open and venting into a small plastic tank, the (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 623psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 11:25a.m.

2575326 Primary Facility 2017-03-24 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection of the Abbott D Well Site. The E&S Plan was on site and the E&S Inspection log indicated that Sam Wiernusz conducted the most recent inspection on 3/13/17.

Operator, please be advised that 102.4(b)(5)(x) states "A maintenance program which provides for the operation and maintenance of BMPs and the inspection of BMPs on a weekly basis and after each stormwater event, including the repair or replacement of BMPs to ensure effective and efficient operation. The program must provide for completion of a written report documenting each inspection and all BMP repair, or replacement and maintenance activities."

2617174 Primary Facility 2017-06-15 Routine/Complete Inspection No Violations Noted I arrived on site to conduct a Routine Complete Inspection. At the time of my inspection, I reviewed the E&S Inspection Log to find that Anne Bonczek conducted the most recent inspection on 6/14/17.
2627721 Primary Facility 2017-08-21 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:20a.m.

The cellar was full of water with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into a small plastic tank, the (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 520psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:00a.m.

2661621 Primary Facility 2017-11-14 Routine/Complete Inspection No Violations Noted On November 14, 2017 I conducted a routine E&S inspection of the Abbott well pad and tank farm. The site as a whole was in good shape with strong vegetation and a solid pad surface which was not muddy. There were no erosional issues apparent.

There were no violations noted.

2668894 Primary Facility 2017-12-08 Routine/Complete Inspection No Violations Noted I inspected the Abbott well site and tank farm on December 8, 2017. I performed a Routine Complete Inspection (RTNC) of the well labeling and erosion and sediment controls. The access road was solid and stable. The cut and fill slopes were well vegetated and the pad surface was solid. The vegetation on site was lush and full. The wells on site (5h and 11h) were properly labeled. There were no erosional issues apparent at the time of inspection.

There were no violations

2738314 Primary Facility 2018-06-06 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 1' below grade in the cellar and there was no bubbling noticed. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2763276 Primary Facility 2018-08-13 Routine/Complete Inspection No Violations Noted I performed a routine complete inspection of the Abbott well site. The well site was solid and stable despite recent monsoon like conditions. The slopes were stable and well vegetated. The access road was stable as well. There was extensive puddling however the recent rainfall has been extensive. The E & S log was up to date. The wells were properly labeled and there did not appear to be any erosional issues at the time of inspection.

There were no violations.

2802443 Primary Facility 2018-10-09 Drilling/Alteration No Violations Noted I arrived at the site at 13:31. Precision Drilling Rig #532 is at the location. I met with Jerry Dugas, Drilling Superintendent of Cabot Oil and Gas, and Aaron Wilhite, site company man, and completions consultant of Cabot. Precision Drilling Rig #532 is at the site in the process of drilling the D Abbott 3H well. The current drilling depth is (3,000) feet. Cabot submitted a Drilling Resumption After Break of 30 days or more notification to the Department on 10/7/18. The comments in the notification indicate Cabot estimated resuming drilling operations on the D Abbott 3H well with Precision Drilling Rig #532 at 12:00 PM on 10/8/18. The surface and intermediate casings have been cemented on the 3H well. The 7H well was the most recent well drilled to total depth. The vertical section of the 3H well is being drilled with water based drilling mud. The horizontal section of the well will be drilled with diesel based drilling mud, which is oil based mud mixed with diesel fuel. I departed from the site
2815935 Primary Facility 2018-12-13 Routine/Complete Inspection No Violations Noted I was originally onsite to perform an inspection in response to a release which occurred onsite. While onsite, I also performed a RTNC E&S inspection of the site.
    The weather was approximately 31 degrees F. and overcast.  The site signage was legible and up to date, as was the E&S log.
    The access road and pad surfaces were solid and stable, as was the cut and fill slopes of the tank and well pads.  The vegetation was visible through the snow and appeared healthy.  There were no erosional anomalies noted during the inspection. 


There were no violations related to E&S issues noted.

2849183 Primary Facility 2019-03-06 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was empty and there was no gas detected. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 3/4/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
