2404233 Primary Facility 2015-09-02 Preoperation Inspection No Violations Noted On 9/2/15, the Department conducted a Preoperational inspection of the Cabot Lambert R Pad located in Harford Township, Susquehanna County. The well pad is approximately 85% complete with all E&S BMP’s installed per plan. Williams is actively installing the well connect line for this pad. Pipeline activity to include a trenching machine has almost the entire east side of the pad disturbed. Any damage which occurs to the perimeter BMP’s, i.e. silt sock damage, etc. will need to be maintained and or replaced. No violations noted. The well permits that have been issued for the pad are: Lambert R 1H(115-22043), 2H(115-22044), 3H(115-22045), 4H(115-22049), 5H(115-22050), 6H(115-22051), 7H(115-22046), 8H(115-22047), and the 10H(115-22048).

2427153 Primary Facility 2015-11-16 Routine/Complete Inspection No Violations Noted Precision Drilling Rig #548 is currently in the process of drilling the R Lambert 1H with water based drilling mud well at a depth of (1,850) feet. Joel Franks of Cabot Oil and Gas is the Company Man on location. A request for approval of an Alternative Waste Management Practice (OG-71) needs to be submitted to the Department for this process. The request for an OG-71 approval encompasses both the use of centrifuges for separation of solid and liquid material in the drill cuttings during oil and water based mud drilling. I noticed some soap content in water on containment accumulated from recent rainfall events. Mr. Franks stated that he would have the water with the soap suds in it removed from containment. The soap is most likely used during water based mud drilling.

2462133 Primary Facility 2016-03-01 Incident- Response to Accident or Event No Violations Noted Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas contacted me at 10:34 AM on 3/1/16 to report the discovery of staining on the pad surface along with an odor of diesel fuel. The staining and odor was discovered on 2/29/16 as Cabot had removed the liner used during drilling operations. Mr. Watson stated that on 1/28/16 an estimated (3) barrels of diesel based drilling mud was recovered from secondary containment. The mud was released through a hole in a Diaz half round container. The Department was not notified of the spill to containment that occurred on 1/28/16.

Last week Cabot discovered dark staining on the liner used during drilling as they were removing rig mats set on top of the liner. As the liner was pulled from the pad surface yesterday around 2PM, staining was discovered on the pad surface due to compromised liner integrity. A smell of diesel fuel was detected as the liner was removed from the pad surface. Spots of dark staining on the pad surface were observed

2463727 Primary Facility 2016-02-29 Routine/Complete Inspection No Violations Noted The main intent of the intent of the inspection was to assess the site as Precision Drilling Rig #548 moved off the location last month and to locate the general areas on the pad surface where Rick Croll, PADEP Radiation Health Physicist, performed radiation monitoring of containers and the containment area the containers were staged on. On 2/22/16 a Diaz Roll-off container which contained drill cuttings, set off Technologically Enhanced Normally Occurring Radioactive Material Alarms at Keystone Landfill in Dunmore, PA.Mike Pirner, Construction Foreman of Cabot Oil and Gas, informed us that residual mud staining was observed on the liner last week while Cabot was in the process of removing rig mats which are used during drilling. He further stated that the containment will be pulled from the pad surface around 2PM today in order to assess the pad surface beneath it and the plastic sheeting was installed next to the liner to prevent the potential of the mud from flowing off the liner to impact the pad surfa
2485685 Primary Facility 2016-05-18 Routine/Complete Inspection No Violations Noted
2537292 Primary Facility 2016-01-04 Incident- Response to Accident or Event No Violations Noted Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas contacted me at 10:31 on 11/4/16 to report an estimated spill of (10-15) gallons of hydraulic fluid to the tank pad surface of the R Lambert well site. The complaint details in the Department's complaint inspection form state the following information. "R. Lambert Pad – Hydraulic hose broke on Susquehanna Gas Field Services water truck. 10 to 15 gallons of hydraulic fluid spilled to the tank pad surface. Spill happened at 9:45 AM Nov. 4, 2016. Backhoe on site to clean-up and roll-off container is on its way. The impacted area will be scraped. Resource Environmental will take PID readings." Mr. Watson stated to me during the phone call that hydraulic fracturing is in progress at the site and the roll-off container should be there around 10:45. He also stated that Cabot will not collect samples; the PID readings will be sufficient enough to determine that the spill is properly cleaned up.

2551018 Primary Facility 2017-01-10 Routine/Complete Inspection No Violations Noted On 1/10/17, the Department conducted an inspection of the COG Lambert R Well Pad in response to COG’s self-reporting of a small spill at this pad. On 1/9/17 at approximately 3:44 PM, Steve Watson, Cabot Senior EH&S Specialist contacted Jon Sassi, Department EPS to report a small spill of hydraulic fluid at this pad. Mr. Watson stated that while conducting trenching activities on the pad, the trenching machine blew a hydraulic hose which caused approximately 6 gallons of hydraulic fluid to be released to the pad surface in a spray pattern. During the inspection, the impacted area was observed and no residual fluid was noted. During the inspection it was noted that the impacted area had been scrapped up and that the impacted pad material was placed into a roll off container for disposal. Resource Environmental is in route to review the remediation and is bringing a PID meter to verify that remediation has been successful. 2556003 Primary Facility 2017-01-27 Administrative/File Review No Violations Noted On 1/23/17, the Department received a Remediation Summary Report from Mr. Steve Watson, Senior EH&S Specialist for the remediation of the 6 gallons of hydraulic fluid which was released on 1/9/17 when a line broke on a trenching machine during installation of permanent production facilities on the COG Lambert R Pad located in Gibson Township, Susquehanna County. The Department has reviewed the report and finds that, based in the information provided the remediation to have been successful. No further action is required of Cabot at this time. Attached is a copy of the report.
2574646 Primary Facility 2017-03-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 8:50a.m.

The well was recently completed and is in production. The well tag was on the well and correct. The cellar had ice at the bottom. The (20x13) annulus vent pipe was open and venting into a small plastic tank, the (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. The well was producing through a separator and the production fluid was going into a 500bbl flow back tank that was on containment. The containment appeared to be in good condition. There was 806psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:55a.m.

2585496 Primary Facility 2017-04-21 Routine/Complete Inspection No Violations Noted Inspection conducted on 4/19/17. Almost all of containment has been removed, except for frac tank containment. Temporary flowback tank operations set up with water truck on-site actively removing flow back water as shown in photo 6. Last weekly inspection conducted on 4/18/17 by Robert Foster. No violations noted.
2669096 Primary Facility 2017-12-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The (20x13), (13x9) and (9x5) annuli vent pipes were all connected together and venting into the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2747582 Primary Facility 2018-06-29 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a restoration inspection. This inspection was conducted to confirm Cabot's assessment that the project meets the minimum requirements for permit termination. The Department’s determination is that the site meets the requirements for permit termination. I recommend the NOT be approved.
2771864 Primary Facility 2018-09-06 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 2' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
