2513770 Primary Facility 2016-08-24 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:45a.m., I met with company man Eric Draxten, SDC Rig 44 was on site and drilling the hole for the production casing. The top hole section of the well was drilled and completed on 9/20/15. We took a walk around the location and everything looked OK.

The depth at time of inspection was 11104'. The projected TD is 14134'. I left the site at 10:45a.m.

2578053 Primary Facility 2017-03-23 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection. At the time of the inspection, the E&S Inspection Checklist Site Inspection Log indicated that SRT conducted the most recent inspection on 3/17/17 and no attention needed.
2635793 Primary Facility 2017-09-13 Routine/Complete Inspection No Violations Noted At time of inspection.

I conducted a routine inspection for an inactive status request received on 9/12/17. The well was drilled to TD on 8/26/16 but has not been stimulated. There was cement between the (13x9) annulus and water just above the casing with no bubbling noticed. There were no lines going to or from the well with a TA cap on the top. All the valves on the well were closed. There were nine wells on the site. The 1H (115-22036), 2H (115-22037) and 4H (115-22038) have not been stimulated. The 3H (115-21226), 5H (115-21926), 6H (115-21821), 7H (115-22039), 8H (115-22040), and 9H (115-22041) are in production. I took a walk around the location and there were no obvious concerns noticed.

2637601 Primary Facility 2017-08-14 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection. At the time of arrival, I reviewed the E&S Inspection Log which indicated that the most recent inspection was conducted on 8/7/11. On site, several 21,000 gallon tanks are situated on containment. Currently the King 9H, 7H, 8H, 5H, 6H, and 3H appear to be in production, while the 1H, 2H, and 4H are present but do not appear to be on line. The access road has sediment built up in areas along the edge, in the roadside ditches, culverts, and rip rap/sump areas. Sediment has also fanned out from the access road rip rap apron/sump. Operator, please maintain the features mentioned per your approved E&S Plan.

Met with company man Aron Lash. Halliburton was on site to frac the 1H, 2H and 4H wells. They were on stage 24 out of 32 stages on the 2H when I arrived on site. The 4H was completed with 32 stages. The 1H hasn't been started yet but will have 34 stages when completed. The water is being pumped to the location from the Young fresh water impoundment and Zeffer production water impoundment. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2705802 Primary Facility 2018-03-14 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The well tag was on the well and correct. There was containment still around the well and was unable to check the cellar. There were 25-500bbl water tanks and a 500bbl flow back tank on containment from a recent frac on the 1H, 2H and 4H wells, the containment was snow covered. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2809821 Primary Facility 2018-12-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 1.5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
