2472173 Primary Facility 2016-03-29 Routine/Partial Inspection No Violations Noted The Department was on site at the Arnone Well Site located at 746 Zicks Hill Road in Brooklyn Township, Susquehanna County. The E&S Inspection log indicated that a post-rain event inspection was conducted on 3/29/16. The site appears to meet temporary stabilization requirements. There is an area around the road crossing where it appears as though some unstabilized material is situated at the outlet side of the culvert. Please review this area and make any necessary changes to ensure compliance.
2482237 Primary Facility 2016-05-09 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:30 a.m. for a new spud inspection, United Drilling was on site to drill and run the conductor pipe for the 1H, 5H, 8H, 9H, and 10H. I met with Brett Wambold of Selman and Associates, LTD, the 5H, 8H, 9H and 10H were drilled to TD at 94' and the rig was drilling the hole for the 1H. A second rig was running the conductor pipe into the wells that were drilled to TD. I took a walk around the location and everything looked OK.

I left the site at 10:00 a.m.

2585034 Primary Facility 2017-04-20 Routine/Complete Inspection No Violations Noted Inspection conducted on 3/30/17. Entire site is stable. Site is in construction phase of layout. Sediment basins and tank pad present. Good temporary stabilization. No violations present.
2595838 Primary Facility 2017-05-15 Routine/Complete Inspection No Violations Noted I arrived at the site at 11:45. The weather conditions were partly sunny skies and breezy with temperatures in the low to mid 50's. Cabot submitted a stimulation notification to the Department on 5/12/17. The notification indicates that stimulation of the Arnone 1H, 4H, 5H, 6H, 8H, 9H and 10 H wells would commence on 5/16/17 at 4:00 AM. I met with Steve Barrett, completions consultant, Steve Brominski, Water Management Coordinator and Ken Wilson completions consultant all of Cabot Oil and Gas. Mr. Barrett stated the following. “We are in the process of rigging up for stimulation operations. Stimulation of the wells is scheduled to commence on 5/17/17. There are a total of (7) wells to be stimulated." Frac operations are set be completed by 6/30/17 according to Mr. Wilson. Mr. Brominski stated the following. "The pugmill unit on the tank pad will continue to process wastewater generated at our well sites during hydraulic fracturing (stimulation) operations. There has not been any processing in a few days now.
2600838 Primary Facility 2017-05-31 Routine/Complete Inspection No Violations Noted
2647969 Primary Facility 2017-08-15 Incident- Response to Accident or Event No Violations Noted Today's inspection, 8/15/17 was conducted in response to the self reporting by Cabot to the Department of a small spill on this pad. The spill occurred while crews were disassembling a temporary waterline. Present during the inspection was Wayne Wilridge, Cabot Consultant. A roll off container, as shown in photo 3 is still on-site and contains the impacted pad material. The remediation appears to have been adequate. Resource Environmental is preparing a spill remediation report for this incident. The drilling liner is being actively removed. No violations were noted during this inspection.
2652286 Primary Facility 2017-10-25 Routine/Complete Inspection No Violations Noted Arrived on location at 10am. 7 wells on location, 7 treaters and 7 separators. All cellars filled with water, however no visible bubbling. Prod casing = 800psi. No violations noted at time of inspection.
2658217 Primary Facility 2017-10-19 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:43 PM. The weather conditions were sunny skies with temperatures in the low to mid 60's, with light winds. I met with Wylie Roof, field supervisor of Susquehanna Services on the tank pad section of the site. Mr. Roof informed me the following in reference to site activities. "Today, trucks offloaded a total of (18.5) barrels of fluid reclaimed. There were (4) trucks that mobilized to the site as of 12:58 today, and (25 tons) of solids per the (4) trucks. Drill out sand is brought here from the Cabot Jeffers Farms Pad 5 and frac tank clean out sand comes from the Cabot Romeika Pad." The drill out sand was most likely generated during drilling out of hydraulic fracturing plugs from the wells on the Jeffers Farms Pad and the frac tank clean out sand transported from the Romeika pad was most likely generated during flowback of the wells. I departed from the site at 13:38.
2670718 Primary Facility 2017-12-04 Routine/Complete Inspection No Violations Noted I arrived at the site at 11:32 in response to a reported spill of (12) gallons of diesel fuel that occurred on 12/2/17. The weather conditions are partly cloudy skies with temperatures around 40 degrees. I met with Wayne Wilridge, Safety and Regulatory Compliance and Ken Wilson, EHS Completions Foreman, both consultants of Cabot Oil and Gas. Mr. Wilson and Mr. Wilridge informed me of the following details associated with the spill. "We were first notified of the spill at 1:40 PM on 12/2/17. The spill occurred as a result of a fuel cap not secured on the tank in the back of a pickup truck. The truck picked up fuel in a (1,000) gallon in capacity above ground fuel cell on the Foltz pad where Keane is conducting the frac. The truck was headed to the Arnone to fuel generators and heaters on the tank pad section of the site. Diesel fuel overflowed the tank onto the truck bed and flowed out of the corner of the tailgate to the Foltz pad access road near the pad entrance, on the entire length of the Foltz pad acces
2670777 Primary Facility 2017-12-06 Routine/Complete Inspection No Violations Noted I arrived at site at 11:57. I was accompanied by Steve Brokenshire, Environmental Group Manager of Oil & Gas Planning and Program Management. The weather conditions were sunny skies and breezy with temperatures in the 30's. Cabot personnel at the site accompanying the Department during the inspection included Jimmy Crouch, Ethan Greenley and Steve Watson. Cabot consultants included Larry Fulmer, Herb Swiney and Wayne Wilridge. Wylie Roof and John Yarosz represeted Susquehanna Services, the subcontractor of Cabot operating the pugmill processing unit. The above mentioned personnel that accompanied the Department during the inspection stated the following. "Frac is still in progress on the Foltz pad. It will be completed in the next few days. All wells on this pad are in production. Diaz brought the last vac box tank from our JHHC pad yesterday. The tank contained wet sediment/sludge generated from flowback of the wells at the JHHC. We recovered (7) barrels of fluid and (18) tons of sand from the pugmill. We a
2734440 Primary Facility 2018-05-03 Follow-up Inspection No Violations Noted Arrive at the site at 13:05. I was accompanied by Gene Rickard, DEP O&G WQS during the inspection. The weather conditions were partly sunny skies, breezy, warm and humid with temperatures in the low to mid 80's. There was no activity or personnel at the site during the inspection as we arrived. The main intent of the inspection was to investigate the impacted area on the tank pad surface where remediation of a spill of flowback fluid residuals and oil from a frac tank was in progress during an inspection conducted by the Department on 5/1/18, and to investigate the impacted area on the tank pad surface due to a spill of flowback fluid residuals from a vacuum box tank. Both spills occurred on 5/1/18. The spill areas are a relatively short distance away from one another. Neither tank associated with the spills was staged on secondary containment. Cabot informed the Department during the 5/1/18 inspection that the tank pad surface impacted due to flowback fluid residuals leaking out of a vacuum box tank,
