2471258 Primary Facility 2016-04-05 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:15 p.m., met with the company man Rafael Rodriguez, Nine Energy Service was running a plug and perforation on a wire line into the 4H. Pumpco Services was on site and getting ready to stimulate stage 6 on the 3H. I took a walk around the location and everything looked OK.

The 3H-had 5 stages completed out of 19 and was stimulating stage 6 when I left. The 4H-had 1 stage completed out of 23. The 7H-had 5 stages completed out of 32. The 8H-had 5 stages completed out of 30. I left the site at 13:30 p.m.

2487474 Primary Facility 2016-05-12 Routine/Complete Inspection No Violations Noted I conducted an inspection of the site accompanied by Briana Cunningham, PADEP O&G WQS. Cabot submitted stimulation notices to the Department on 3/31/16 indicating hydraulic fracturing operations would commence on 3/31/16. We met with Don Griffith, site company man. Mr. Griffith informed us the following. "Flowback operations commneced at 5AM, today, 5-12-16. The S Warriner 3H, 4H, 7H and 8H wells are in the process of being flowed back simultanoeusly. Erick Flowback Services is the Cabot subcontractor conducting the flowback process. At this point, the wells are only flowing back water, no gas has been encountered. Rain for Rent frac tanks on containment in the tank battery in the tank pad section of the site are not being used to contain flowback water from the wells. The tanks were used during hydraulic fracturing. Flowback water is being stored in gas buster and frac tanks on containment near the wells. The other work crew on-site is in the process of setting up the wells for production."

2497401 Primary Facility 2016-06-29 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:25a.m., the wells were in production with the production line going through a sand separator and the fluid going to a five hundred barrel gas buster tank. The cellar had water at the bottom with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 533psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 12:05p.m.

2548776 Primary Facility 2016-12-12 Follow-up Inspection Pending The Department was on hand to conduct a Follow Up inspection of the Warriner Pad (ESX29-115-0011) The wells associated with this pad are as follows:

The Department, represented by Jon Sassi and Frank Hoehle were previously on site December 9, 2016 to inspect a release which occurred December 8, 2016.

The brine tanks which had issues had been switched out with other tanks on site, and there was fresh gravel under all brine tanks on site. All piping to tanks had evidence of recent maintenance (fresh teflon tape) and there were no unusual conductivity readings surrounding tank area. The rolloff container which had been on site has been removed and the tank from which the release occurred now sits idle at the east side of the tank battery. All residue of release has been cleaned up and removed from site.

2586475 Primary Facility 2017-04-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m.

The cellar had water at the bottom with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into a small plastic tank, the (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 507psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:30p.m.

2661644 Primary Facility 2017-11-14 Routine/Complete Inspection No Violations Noted On November 14,2017 I inspected the Warriner S P1 pad for a routine E&S inspection. The pad was located at the end of a relatively long access road which was solid and stable. The pad itself was solid with a bit of ponding due to recent rains. The fill slope and all vegetated areas were in good shape and well stabilized. There did not appear to be any erosional issues at the time of inspection.

There were no violations noted.

2693013 Primary Facility 2018-02-08 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2792495 Primary Facility 2018-10-22 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 6' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
