2412732 Primary Facility 2015-10-05 Drilling/Alteration No Violations Noted Remarks: At time of inspection, arrived on site at 12:30 p.m., met with company man Perk McDowell, H&P Rig #371 was drilling production hole with water based mud with salt and polymers added, the polymers were (XG-VIS, Ener Pac Regular, and Ener Pac Lo Vis), depth at time of inspection was 10956', pressure on the (13x9) was 0psi after a 12hr shut in on 10/2/15, 3.4% LEL on the (13x9) at time of inspection at the poly tank, took a walk around the location and everything looked OK.
2417498 Primary Facility 2015-10-21 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 11:00, met with company man Sam Morgan, H&P Rig #371 was pulling drill pipe, once the drill pipe is out they plan on running back in to take core samples, depth at time of inspection was 13488'. The rig lost circulation at 11425' and 11625', they ran a cement plug from 10900' to 11800', when they went back in to drill they hit hard cement at11430' and drilled out of cement at 11830'. There was no change on the (13x9) annuli, pressure and flow readings are taken once a week, took a walk around the location and everything looked OK.
2419799 Primary Facility 2015-10-28 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 12:40, met with company man Sam Pierce, H&P Rig #371 was rigging up a fishing tool (7.5" cut lip with an overshot bowl) to fish out wire line and logging tool. The logging tool is hung up at 11,890'. There was no change on the (13x9) annuli, pressure and flow readings are taken once a week, took a walk around the location and everything looked OK.
2421399 Primary Facility 2015-11-02 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 12:40, met with company man Sam Pierce, H&P Rig #371 was rigging up the BHA. Since the last inspection the logging tool was fish out of the hole and three cement plugs were set. The top plug was run on 11/1/15 at 19:45. The rig will run tools in the hole and load the hole with oil based mud but will not start drilling until after the 24hr wait time on the cement. There was no change on the (13x9) annuli, pressure and flow readings are taken once a week, took a walk around the location and everything looked OK.
2511287 Primary Facility 2016-08-16 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:00a.m., Keane was on site and fracturing the wells, I met with company man Larry Fulmer. They were fracturing stage 13 out of 35 on the 3H and running a plug and perorating on the 6H.

They have completed: 13 stages out of 35 on the 1H, 12 stages out of 30 on the 2H, 12 stages out of 34 on the 5H, and 13 stages out of 29 on the 6H. They were fracturing with fresh water and recycled water, all the tanks were on containment and the containment looked in good condition. I took a walk around the location and everything looked OK. I left the site at 11:50a.m.

2519977 Primary Facility 2016-09-09 Routine/Complete Inspection No Violations Noted I met with Carl Potter, site company man and completions consultant of Cabot oil and Gas. There are (2) coil tubing units on the location in the process of drilling out hydraulic fracturing plugs. One of the coil tubing units is owned by Cudd Energy Services, while the other is owned by Legend Energy Services. Cabot submitted a stimulation notice to the Department on 8/3/16. The notice indicated that pressure test surface iron to begin frac operations on the Halliday 1H, 2H, 3H, 5H and 6H on 8/4/16 at 8AM. There are no wells in production. Mr. Potter stated the following. "Frac plug drill outs commenced on 9/7/16. The plugs are currently being drilled out on the 2H and 5H wells. In the southeast section of the well pad near frac tanks used as working tanks during hydraulic fracturing operations, I observed (2) small spots of dark staining on the ground surface. The dark staining was indicative of a petroleum substance released to the pad surface. I observed a significant amount of what appears
2534038 Primary Facility 2016-11-02 Follow-up Inspection No Violations Noted At time of inspection, arrived on site at 11:25a.m., the wells were recently completed and put into production. I met with well tender Matt Faux, when the 1H was drilled; Cabot notified the department on 9/23/16 that gas was detected at the (13x9) annuli. The cellar had water at the bottom with no bubbling noticed. The (20x13) vent pipe was open and venting into a tank, the (13x9) vent pipe was open and venting into a tank, the (9x5) vent pipe was open and venting into a tank. All the vent lines are connected together before going into the poly tank. Matt unlocked the valves and closed the valve going to the poly tank and then he opened the valve so the (13x9) annulus could be checked for gas. I used an Altair 5X gas meter, I got a reading that peaked at 13% LEL then dropped to 0% and 0% methane, I reset the meter and then got a reading that peaked at 8% LEL then dropped to 0% and 0% methane. It was determined that the LEL reading was from cross contamination. There was 803psi on the production casing. I to

2622693 Primary Facility 2017-08-04 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:05a.m.

The cellar had water at the bottom with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into a small plastic tank, the (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 631psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:55a.m.

2730573 Primary Facility 2018-05-16 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and venting to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2787199 Primary Facility 2018-10-02 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Halliday well site.

The day was warm with intermittent rain, heavy at times.

The wells were properly labeled.

The access road and roadside ditch were solid and stable as were all slopes leading to the well pad. The well pad cut and fill slopes were well vegetated, in excess of the 70% required by the Department. The vegetation was lush, healthy and perennial.

There were no erosional issues noted.

There were no violations noted.

2831742 Primary Facility 2019-01-24 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water and ice 7' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
