2500731 Primary Facility 2016-07-12 Routine/Complete Inspection No Violations Noted On 7/12/16, the Department conducted an inspection of the Cabot Decker T Well Pad located in Harford Township, Susquehanna County. The inspection involved reviewing the fill section of the pad that Williams is actively installing pipeline. This section of the pad looked good. Recent heavy rain events showed no impacts below the pads E&S and storm water controls. Fracking operations were observed taking place. Sand trucks were observed actively entering and leaving the pad. Water is being delivered to the pad via an above ground water line which is coming off the tank pad which has a battery of tanks on-site, located about an 1/8 of a mile away. No issues were observed from the installation and use of this line. Joe Gaboff, Department OGI was on the pad and noted that fracking of the 3H was being conducted at the time of his inspection. The other wells on this pad that are presumably being stimulated are the 1H, 2H, 5H, and 7H. No violations noted.
2507118 Primary Facility 2016-08-02 Routine/Complete Inspection No Violations Noted On 8/2/2016, the Department conducted an inspection of the COG Decker T Pad located in Harford Township, Susquehanna County. A crew from Stallion Oilfield Services is on-site conducting flow back operations. As of the inspection, the 1H and the 2H have been flowed back and are ready for production. The 3H, 5H, and the 7H are also slated to be prepped for production. Recent rain events have left some standing water on all four quadrants of the pad. Conductivity testing of the standing water in all four quadrants found no indicators that a release has occurred. During the inspection crews were actively vacuuming up excess water from almost all containment on the pad. Pad stability looks good. Pipeline tie-in to the pad looks stable. A review of the tank pad revealed all tanks and containment have been removed. Conductivity readings were also relatively low on this pad. No violations noted. The poly line has been removed that connected the tank pad to the well pad.

2516404 Primary Facility 2016-08-29 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:50a.m., I talked to well tender Jeremy Rapp and he told me the wells were turned into production on 8/22/16. They were still producing through sand separators and the fluid was going into a 500bbl flow back tank at about 34bbls per hour, the fluid was being hauled to Comtech. The cellar had water at the bottom with no bubbling noticed, the (13x9) vent pipe was open and venting into a tank, the (9x5) vent pipe was open and venting into a tank, there was 536psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 10:45a.m.

2516775 Primary Facility 2016-08-30 Routine/Complete Inspection No Violations Noted On 8/30/16, the Department conducted an inspection of the COG Decker T Pad located in Harford Township, Susquehanna County. Good stabilization of the site exists. E&S BMP’s still emplaced and in good condition. Perennial vegetation appears to be well established. All 5 wells associated with this well pad have been drilled, stimulated, and appear to be in production. Most drill rig/stimulation containment has been removed. Permanent production equipment has been installed and is operational. A frac tank is located next to the permanent production fluid containment, and appears to be present to increase holding volume of initial produced fluid. 2 large roll off containers on-site, on containment with sawdust in each presumably being utilized for bulking purposes. Large number of frac tanks(20ish) still on pad on containment. A small crew was on-site removing temporary buried piping. No visible sign of a release was observed. No violations noted.
2533806 Primary Facility 2016-11-01 Routine/Complete Inspection No Violations Noted On 11/1/16, the Department conducted an inspection of the COG Decker T Well Site located in Harford Township, Susquehanna County in response to a nearby malodor complaint. The Department arrived on-site at approximately 11:00 AM. There are 5 wells drilled and in production on this pad, the 1H, 2H, 3H, 5H and the 7H. The pad is equipped with the standard equipment associated with steady state production. There was also a backhoe and a utility truck parked on the pad. No other activity is occurring at this pad. I spent approximately 5 minutes at each corner of the well pad and did not detect any malodors. No E&S issues were observed during this inspection. No violations noted.
2612744 Primary Facility 2017-07-06 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m.

The cellar was about a quarter full of water with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into a small plastic tank, the (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 520psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:25p.m.

Well pad and access road have been plowed. No obvious E&S issues noted during the inspection. All wells drilled on pad appear to be in production. No violations noted.

There was water 7' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar with a MSA Altair 5X multi gas detector that was calibrated on 12/3/18. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 12/3/18. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
