2363739 Primary Facility 2015-03-19 Preoperation Inspection No Violations Noted On 3/19/15, the Department was on site at the Cabot Colwell A. Pad 1 to conduct a Preoperational Inspection. The well permits that have been issued for this pad are the Colwell 1H (115-21885), the 2H (115-21886), and the 5H (115-21887). All perimeter BMP’s in place to include red lined additions. Pad is 100% complete with about 20% of the pad left to mulch. No violations noted.

2376782 Primary Facility 2015-04-20 Routine/Complete Inspection No Violations Noted On 4/20/15, the Department conducted an inspection of the Colwell A. Pad in response to a reported small spill of diesel fuel. Steve Watson of Cabot Oil and Gas reported that on 4/19/15, at approximately 11:50 pm, a truck delivering barite to the Colwell Pad had leaked diesel fuel onto the access road of the pad. The delivery truck fuel tank cap became dislodged and while the truck was moving up the access road, which has a relatively steep grade, the fuel spilled out as it sloshed around in the tank, consequently splashing out of the fuel tank onto the road surface.
2434434 Primary Facility 2015-12-15 Routine/Complete Inspection No Violations Noted Flowback operations and setting up for production of the 1H, 2H and 5H wells were in progress as we arrived to the site at 11:00. Conductivity of water accumulated from recent rainfall events in containment was (1.38) milliSiemens/cm, where the Erick Flowback Services gas buster tanks are staged, (2) ms/cm in the Rain for Rent spill guard containment next to a pump in the frac tank battery in the Southeast corner of the pad, (2-3) ms/cm in the tank battery and pad surface in the Southwest corner of the pad near where heavy equipment is staged and a trench has been dug in the pad surface to eventually contain production pipeline. The elevated conductivity readings indicate the potential that hydraulic flowback fluid may have been spilled to containment and has been diluted with rainwater in these particular areas of the site. Petroleum sheen was observed in the Rain for Rent Spill Guard containment near the port-a-johns where (4) drums are staged. One of the drums was labeled Oil & Diapers, Colwell 11/20/15.

The (20x13) vent pipe was open and venting into a small plastic tank, the (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 683psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:20p.m.

2643623 Primary Facility 2017-09-12 Routine/Complete Inspection No Violations Noted 3 wells located on pad. Equipment onsite includes: 1 brine tank, 3 glycol tanks and 1 condensate tank. All cellars filled with water with no visible bubbling at time of inspection. Production casing obtained from heater treaters are as follows: 1H-786.86psi; 2H-810.04psi; 5H-775.51psi. No violations noted at time of inspection.

The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
