I met with company man Sam Pierce and observed the cement job, I took a walk around the location and everything looked OK.

I met with company man Perk McDowell, we took a walk around the location with fellow DEP inspector Jon Sassi, we saw one rip in the containment and two spots of sheen on a different part of the containment. Perk said he will have them taken care of. Depth at time of inspection was 5244', projected TD for the 5H is 10980'.

Everything else looked OK. Depth at time of inspection was 10412', anticipated TD is 12048'.

On Friday, January 20th, a report of mud on the road (SR167) near the B. Mead well pad was called in by the Susquehanna County Conservation District. This was investigated by Frank Hoehle of the Department of Environmental Protection, and at the time, it was observed that there was mud on the road, but it was in the process of being swept up and the Rock Construction Entrance of that well pad was being bolstered by additional stone. The mud on the road was extending down the road and was also a problem at the Diaz well pad entrance, but was also being taken care of at the time. An additional follow up inspection by Briana Cunningham on the following Monday, January 23rd, 2017 revealed a larger amount of mud on SR167, and some follow up emails from Steve Watson informed the Department tha

2584948 Primary Facility 2017-04-20 Routine/Complete Inspection No Violations Noted Todays inspection, 3/23/17 is a routine inspection. No one was present during the inspection. All six wells appeared to be in production. Well pad still snow covered. No violations observed. The access road has an extremely steep grade.
