2430341 Primary Facility 2015-12-02 Routine/Complete Inspection No Violations Noted H&P Drilling Rig # 371 is currently in the process of drilling the Diaz Family 5H well with water based drilling mud well at a depth of (5,240) feet. The containment was torn near the drilling rig and hydraulic lift for the drill pipes. The ground surface was revealed below the felt and plastic liners. There were a few petroleum sheens observed on containment. Mr. McDowell stated that the sheens would be removed from containment and the torn containment will be repaired at 1PM this afternoon, 12/2/15. The Preparedness Prevention and Contingency and Plans in the Company Man's trailer were dated 2009 and 2012. Cabot submitted and updated version of the plan to the Department on 4/29/15. The PPC plan also includes a Control and Disposal Plan and Master Containment Plan. Mr. McDowell, the site company man, was not able to locate the updated the PPC plan in the trailer. The plan was not located in the mailbox next to the sign and the access road of the site.

On Friday, January 20th, a report of mud on the road (SR167) near the B. Mead well pad was called in by the Susquehanna County Conservation District. This was investigated by Frank Hoehle of the Department of Environmental Protection, and at the time, it was observed that there was mud on the road, but it was in the process of being swept up and the Rock Construction Entrance of that well pad was being bolstered by additional stone. The mud on the road was extending down the road and was also a problem at the Diaz well pad entrance, but was also being taken care of at the time. An additional follow up inspection by Briana Cunningham on the following Monday, January 23rd, 2017 revealed a larger amount of mud on SR167, and some follow up emails from Steve Watson informed the Department tha
