I took a walk around the location and everything looked OK.

I left the site at 14:10 p.m.

2498879 Primary Facility 2016-06-27 Complaint Inspection No Violations Noted 1900- Spoke to Complainant about the noise and grey/white smoke coming from the C. Fontana well pad. Complainant indicated that a loud hissing noise with grey/white billowing smoke was coming from the C. Fontana well pad. Complainant stated that she called the Cabot emergency number. Cabot informed Complainant that personnel are in the process of relieving pressure from the production equipment. Complainant voiced concerns about the duration (approximately 20 minutes) and the presence of the smoke.

Arrived at the C. Fontana well pad, the gate at the access road was locked. There weren't any Cabot personnel on-site. On the well pad, no noise or smoke was detected. Inspected the C. Fontana 2H, 4H and 6H gas wells. The wells were in production. No leaks were detected at the well heads and no bubbling was observed in the water filled well cellars. The gas production units (GPUs) for the C. Fontana wells were inspected. No leaks were detected. No evidence of spills were observed around the well heads or GPUs. Ins

2498882 Primary Facility 2016-06-28 Complaint Inspection No Violations Noted On 6/28/2016, David Schoonover (DEP-Oil and Gas Inspector) contacted Michael Hall (Cabot- Production Foreman). Mr. Hall indicated that, at the time of the complaint, Scott Allen (Cabot Well Tender) was at the C. Fontana pad blowing down the C. Fontana 4H in an effort to remove fluid from the well. No issues were reported during this process.

2559585 Primary Facility 2017-02-08 Routine/Complete Inspection No Violations Noted The Department was on site at the Fontana well site located at 2639 Post Pond Road Bridgewater Township to conduct a Routine Complete Inspection. I reviewed the E&S Inspection Log which stated that Sam Wiernusz conducted the most recent inspection on 2/2/17 and it was a Post Precipitation Inspection. The site appeared to meet temporary stabilization requirements. Several areas on the site appeared to have been recently seeded and mulched/covered in erosion control blanket.
2600432 Primary Facility 2017-05-30 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected The Department received a report from Cabot Oil and Gas concerning a release that occurred on 5/25/17 at 1700 hours. Mr. Watson contacted the DEP after hours hotline at 1753 and received a return call from Bob Bisignani (DEP). It was reported as less than 1bbl, but the exact volume was not determined. A well was being flowed black into a flow back tank in an attempt to increase gas production on the 2H well. A well tubing hanger was temporarily removed, to increase the diameter of the well, and the well was flowed to a flowback tank. When the well was turned back in line fluid in the gas supply line (not typical) was forced to the GPU.

I met with Mr. Watson on site to conduct the inspection. The inspection was conducted during a heavy rainstorm. .

2626922 Primary Facility 2017-08-16 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:00p.m.

The cellar had water at the bottom with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 487psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:40p.m.

2676727 Primary Facility 2017-12-27 Site Restoration No Violations Noted I performed a Notice of Termination (NOT) inspection on the Fontana well site on December 27, 2017. The site was solid and stable and the slopes were all well covered with last years vegetation, at or in excess of the 70% perennial growth standard required.

The well pad has been reduced in size from approximately 765'x330' to 343'x265'.

The vegetated filter strip PCSM is in place and well vegetated with last years growth.

All wells were properly labeled and had a legible tag The wells associated with this site are as follows: 115-21814 115-21815 115-21816


There were no violations observed.

I recommend APPROVAL of this NOT.

2679074 Primary Facility 2018-01-08 Routine/Complete Inspection No Violations Noted I performed a Notice of Termination (NOT) inspection on the Fontana well site on December 27, 2017. 2737139 Primary Facility 2018-06-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 7.5' below grade with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2833484 Primary Facility 2019-01-30 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of ice and was unable to check for gas. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
