Inspection conducted with Rich Gulyas, SWN Construction Foreman. The well permits that have been issued for this pad are the Breese 1H (115-21789), 2H (115-21790), 3H (115-21791), and the 4H (115-21792). All perimeter BMP’s in place to include red lined additions. Pad is 100% complete. No violations noted.
2454011	Primary Facility	2016-02-22	Routine/Complete Inspection	No Violations Noted	On 2/22/2016, the Department conducted an inspection of the SWN RU40 Breese Pad. The only well that has been drilled and is in production is the Breese 3H. Stabilization looks good. Pad looks clean. No violations noted.
2568255	Primary Facility	2017-03-07	Incident- Response to Accident or Event	No Violations Noted	Frank Hoehle met with Seth Tratthan on Friday March 3 in response to a call from SWN notifying DEP of an erosional event. On Thursday March 2, 2017, upon performing post storm e&s inspections, it was discovered that rainwater from culvert overtopped small retention berm, went off lod, and ran down hill causing washout of slope area to roadside ditch below. Sediment piled up in low spot in ditch. The DEP was notified just before 5pm on March 2. While Frank Hoehle and Seth Tratthan were on site investigating incident and discussing repair, small excavator arrived on scene with operator and laborers to repair damaged area. Discussion by SWN to include rerouting storm water to a less concentrated area of flow is in works currently.
2584651	Primary Facility	2017-04-19	Routine/Complete Inspection	No Violations Noted	The Department was on site to conduct a RTNC inspection on the RU40 Breese well pad. The areas which were inspected during the previous Department visit were all as bad as they were previously. (See photos 4 -11). There was runoff from the site exacerbating the previous erosional issues. In addition, during the site visit, it was also noted there were other new areas of issue. The first is near the entrance to the access road (photo 2). There is also an area of concern farther up the access road where soil is eroding from a vegetated area (see photos 11 and 12). It is recommended these areas be addressed as soon as possible, and perhaps the runoff from the road should be re engineered.
