2353675 Primary Facility 2015-02-19 Routine/Complete Inspection No Violations Noted Precision Drilling Rig #548 is the active drilling rig on the location currently in the process of drilling the A Stellitano 5H well at a depth of (415) feet. While in the Company Man trailer, I requested a copy of The Preparedness, Prevention and Contingency Plan which includes the Control and Disposal Plan. Mr. Franks was not able to locate the plan during the inspection. While at the L Aldrich Pad in response to a flowback fluid spill, I informed Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas of the PPC plan not located during the inspection of the Stellitano Pad. Mr. Watson stated that he would go to Stellitano Pad upon completion of the spill response investigation at the Aldrich Pad to check for the plan. Mr. Watson e-mailed me a picture of Cabot's PPC plan at 5:13 PM on 2/19/15. In accordance with 25 Pa. Code § 78.55(d) Control and Disposal Plan, "A copy of the plan shall be provided to the Department upon request and shall be available at the well site during drilling and completion activities for review".

2440594 Primary Facility 2016-01-13 Routine/Complete Inspection No Violations Noted On 1/12/16, Steve Watson, Cabot EH&S Specialist, contacted the Department to report the release of approximately 25 gallons of blended fluid on their Stellitano Pad. The release occurred at 7:40 PM that day when a hose clamp failed during a procedure to dewater a low point on the overland water line being utilized to transport the blended fluid from the COG Aldrich Pad to the fracturing operations being currently conducted on the Stellitano.

On 1/13/16, the Department met Mr. Watson at the spill site. Two technicians from Resource Environmental were on-site conducting characterization on the impact extent. Thus far it appears that most of the impact occurred on the access road itself. A gross amount of potentially impacted material has been removed and placed on containment. Resource is actively conducting soil conductivity testing as part of the remediation process.

2448723 Primary Facility 2016-02-04 Routine/Complete Inspection No Violations Noted On 2/4/16, the Department received a report from Jim Garner, Susquehanna County Conservation District Manager stating that fracking operations being conducted by Cabot on their Aldrich and Stellitano Pads were causing mud to be deposited on State Route 92 at the intersection of SR 92 and the Great Bend Turnpike. During a site visit, a crew was observed monitoring this intersection and actively cleaning the intersection. No soil was observed on SR 92. This intersection is located approximately ½ mile from the access roads of both pads which are located on the Great Bend Turnpike. Mike Pirner, Cabot Construction Foreman was stationed at the Aldrich pad monitoring operations to include monitoring conditions of the unpaved Great Bend Turnpike. The Department finds that the fracking operations are not directly impacting SR 92. Mr. Garner was directed to contact either PENNDOT or the Township in resolving any future perceived issues at this particular intersection. No violations were noted during the inspec
2453842 Primary Facility 2016-02-23 Routine/Complete Inspection No Violations Noted On 2/23/16, the Department collected water samples from a nearby complainant. The samples were clear with no effervescence observed. The nearest gas well activity/development to the complainant’s water well are the following 3 Cabot well pads:

Reilly J Pad This is the closest pad and is located approximately 2900’ away. There is only one well on this pad, the 3H which is in production. The well was spud on 8/10/2012, drilling was completed on 10/12/2012, and stimulation was completed on 1/10/2013.

Plonski I Pad This pad is located approximately 3500’ away. There are 5 producing wells on this pad. All the wells were spud in June, 2014 with stimulation completed for all three wells in November, 2014.

Stellitano A Pad This pad is located approximately 4000’ away. There are 8 drilled wells on this pad that are currently being stimulated.

There are no violations associated with this complaint inspection.

2469664 Primary Facility 2016-03-30 Routine/Complete Inspection No Violations Noted On 3/30/16, the Department conducted an inspection of the COG Stellitano A. Well Pad. Flow back operations have been completed. Security shack has been removed. Crews are on-site installing production equipment. Pad slopes appear to be stable. No violations noted.
2471546 Primary Facility 2016-04-06 Administrative/File Review No Violations Noted On 1/13/16, the Department conducted an inspection of the COG Stellitano A. Well Pad in response to COG self-reporting of a spill of approximately 25 gallons of blended fluid being utilized in the hydraulic fracturing activities occurring on the Stellitano Pad. Present during the inspection was Steve Watson, Cabot EH&S Specialist and 2 technicians from Resource Environmental. As of today’s date, the Department’s records indicate that a remediation report has not been received from Cabot outlining their technique and presumably their successful remediation of this spill. Jon Sassi, Department Environmental Protection Specialist and I are thus tentatively prepared to conduct our own investigation tomorrow, 4/7/2016 in order to evaluate the remediation of said spill. Any input and/or assistance Cabot may want to offer in the evaluation would be appreciated.
2477580 Primary Facility 2016-04-08 Follow-up Inspection No Violations Noted On 4/8/2016, the Department conducted a follow-up inspection of a release of 25 gallons of blended fluid at the Cabot Stellitano A. Well Pad located in Gibson Township, Susquehanna County. Present during the inspection were myself, Jon Sassi, Department Environmental Specialist, Steve Watson, COG EH&S Specialist, and Matt Forba, Resource Environmental Technician. The Department conducted soil conductivity testing at the location of the release and down gradient of the release. Readings ranged from 0.5 microSiemen/cm³ to 0.005 microSiemen/cm³. The Department considers these reading to be of a normal soil conductivity range for this area. The Department also reviewed the Remediation Summary Report prepared by Resource Environmental and submitted to the Department by Cabot for the remediation of this spill. The Department finds that, based on this report and the field conductivity readings observed by the Department that the remediation conducted by COG to have been adequate and that no further action nee
2553378 Primary Facility 2017-01-18 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:50p.m., the cellar had ice at the bottom. The (20x13) vent pipe was open and venting into a small plastic tank, the (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 603psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

2640682 Primary Facility 2017-09-27 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. All the annulus vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2730496 Primary Facility 2018-05-08 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. No E&S issues were observed. No violations noted.
2746753 Primary Facility 2018-06-28 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 7' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2755124 Primary Facility 2018-07-18 Routine/Complete Inspection No Violations Noted I arrived at the site at 11:27. I was accompanied by Casey Baldwin and Frank Hoehle, DEP O&G WQS's out of the Scranton District Office. The weather conditions are mostly sunny skies, breezy and comfortable with temperatures in the low to mid 70's. There was no activity or personnel at the site during the inspection. The Stellitano A, Pad 1 1H-8H wells are in production. The Department left the site at 11:55.
2756776 Primary Facility 2018-07-27 Administrative/File Review No Violations Noted On 7/26/18 at 8:47 AM, Steve Catalfamo, Professional Geologist and Senior EH&S Specialist of Cabot Oil and Gas forwarded an email to me in reference to an inspection conducted by the Department at the site on 7/18/18, inspection ID# 2755723. Mr. Catalfamo stated the following in the email. "I noticed in the caption of photo #14 of this report, conductivity within the secondary containment for the produced water tank is identified as "749 millisiemens/cm". Was the unit actually microsiemens/cm? Just want to confirm." I had contacted Mr. Catalfamo at 9:07 AM to inform him that I had made an error in the inspection report by indicating the conductivity reading recorded by Casey Baldwin, DEP O&G WQS out of the Scranton District Office, was (749) microsiemens/cm rather than (749) millisiemens/cm, during the inspection on 7/18/18. I had mentioned to Mr. Catalfamo during our phone conversation that the reading may be slightly elevated due to a spill of production fluid to stormwater in the production fluid tank c
2850162 Primary Facility 2019-03-07 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was ice 5.5' below grade in the cellar and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 3/4/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
