The release occurred at 7:40 PM that day when a hose clamp failed during a procedure to dewater a low point on the overland water line being utilized to transport the blended fluid from the COG Aldrich Pad to the fracturing operations being currently conducted on the Stellitano.
On 1/13/16, the Department met Mr. Watson at the spill site. Two technicians from Resource Environmental were on-site conducting characterization on the impact extent. Thus far it appears that most of the impact occurred on the access road itself. A gross amount of potentially impacted material has been removed and placed on containment. Resource is actively conducting soil conductivity testing as part of the remediation process. |

During a site visit, a crew was observed monitoring this intersection and actively cleaning the intersection. No soil was observed on SR 92. This intersection is located approximately ½ mile from the access roads of both pads which are located on the Great Bend Turnpike. Mike Pirner, Cabot Construction Foreman was stationed at the Aldrich pad monitoring operations to include monitoring conditions of the unpaved Great Bend Turnpike. The Department finds that the fracking operations are not directly impacting SR 92. Reilly J Pad This is the closest pad and is located approximately 2900’ away. There is only one well on this pad, the 3H which is in production. The well was spud on 8/10/2012, drilling was completed on 10/12/2012, and stimulation was completed on 1/10/2013. Plonski I Pad This pad is located approximately 3500’ away. There are 5 producing wells on this pad. All the wells were spud in June, 2014 with stimulation completed for all three wells in November, 2014. Stellitano A Pad This pad is located approximately 4000’ away. There are 8 drilled wells on this pad that are currently being stimulated. There are no violations associated with this complaint inspection. |

Present during the inspection were myself, Jon Sassi, Department Environmental Specialist, Steve Watson, COG EH&S Specialist, and Matt Forba, Resource Environmental Technician. The Department conducted soil conductivity testing at the location of the release and down gradient of the release. Readings ranged from 0.5 microSiemen/cm³ to 0.005 microSiemen/cm³. The Department considers these reading to be of a normal soil conductivity range for this area. The Department also reviewed the Remediation Summary Report prepared by Resource Environmental and submitted to the Department by Cabot for the remediation of this spill. "I noticed in the caption of photo #14 of this report, conductivity within the secondary containment for the produced water tank is identified as "749 millisiemens/cm". Was the unit actually microsiemens/cm? Just want to confirm." I had contacted Mr. Catalfamo at 9:07 AM to inform him that I had made an error in the inspection report by indicating the conductivity reading recorded by Casey Baldwin, DEP O&G WQS out of the Scranton District Office, was (749) microsiemens/cm rather than (749) millisiemens/cm, during the inspection on 7/18/18.
