The Well Permits issued for this pad are the Choconut Valley Farms (115-21718, 115-21719). All perimeter BMP's are in place and pad is currently about 70% complete. Wood chip stockpile on site is currently being removed. Rock Sumps along roadway installed and rock being installed during inspection. 2311440 Primary Facility 2014-10-08 Routine/Complete Inspection No Violations Noted The Department was on site at the Talisman Energy Choconut Valley Farms Well Site to conduct a Routine Complete Inspection. The Well Permits issued for this pad are the Choconut Valley Farms (115-21718, 115-21719).
2375453 Primary Facility 2015-06-01 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection of the Choconut Valley Farms Well Pad. There was previously an NOV for this site due to poor vegetation and while the site has low vegetation in many spots still, a contractor is scheduled to repair all of the affected areas, and the site is being monitored for erosional issues. The site is currently stable, due to flexterra on many of the areas of low vegetation still holding together.

2462620 Primary Facility 2016-02-29 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a routine complete inspection of the Choconut Valley Farms well site (ESX29-115-14-0047). The site was stable with no ongoing work at the time of inspection.

There are no erosional issues apparent at this time.

2498595 Primary Facility 2016-07-05 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:40a.m., the cellar was about half full of water with no bubbling noticed, the (20x13) vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) vent pipe was open and venting into the atmosphere with gas detected at 100% LEL and peaked at 66% methane with no measurable flow. There was 1047psi on the production casing and 485psi on the production tubing. The production line was connected to a separator and the bottom line was going to a 500bbl tank that was on containment and the valve was closed. I took a walk around the location and noticed some small stains on the ground, I contacted Talisman Energy and spoke with Kris Perrritt and let him know of the stains and he told me they would get them taken care of. I left the site at 11:15a.m.
2544498 Primary Facility 2016-11-17 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a routine complete inspection of the Choconut Valley Farms well site (ESX29-115-14-0047).

The site was stable and vegetated, however the vegetation on the back side of the retention basin was somewhat light. There was no slippage or evidence of erosion at this time, however it may be recommended to keep an eye on this area to ensure it remains stable. There was evidence of recent repair on some spots of this well pad.

2588462 Primary Facility 2017-04-27 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:50p.m.

The cellar was about three quarters full of water with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) annulus vent pipe was open and venting into the atmosphere with gas detected at 100% LEL and peaked at 26% methane with no measurable flow. There was a 500bbl tank on containment and the containment appeared to be in good condition. There was 615psi on the production casing and 494psi on the production line, the well was producing through a separator. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:25p.m.

2627488 Primary Facility 2017-08-21 Routine/Complete Inspection No Violations Noted I inspected the site on August 1, 2017. The area roads on the way to the well site were all under repair by township and PADOT, As a recent storm had things in the area quite torn up. The access road had a bit of washing evident, but it appears the most extensive damage was the filling on a sediment basin, which is recommended to be cleaned out. The slopes around the access road and well site were all well vegetated and stable with no observed slides or erosion. The pad surface was solid and stable.

There were no violations noted.

2697775 Primary Facility 2018-02-20 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The water was 1.5' below grade in the cellar and there was no bubbling noticed. There was a 500bbl tank on containment that was connected to the bottom of the separators, the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2796366 Primary Facility 2018-10-30 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water .5' below grade in the cellar and there was no bubbling noticed. The (20x13), (13x9) and (9x5) annuli were venting to the atmosphere. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
