All perimeter BMP’s in place to include red lined additions. Pad is approximately 98% complete. No violations noted.
2278009 Primary Facility 2014-05-28 Routine/Complete Inspection No Violations Noted The Department conducted an inspection of the Cabot Plonski I well pad on 5/28/14. The site is approximately 50% complete. Both sediment traps are in place. Perimeter controls appear to be installed properly, but site is currently not developed enough to determine if controls installed in proper locations. No violations noted. This inspection does not qualify as a preoperational inspection.

2294086 Primary Facility 2014-07-21 Follow-up Inspection No Violations Noted Arrived at the site at 12:25 to conduct a Follow-up inspection in relation to a hydraulic fluid spill that occurred on 6/24/14. It was reported that an estimated (20) gallons of hydraulic fluid fluid was spilled to the pad surface. The spill has been reported as cleaned up. The Deparmtent is awaiting a spill report from Cabot.

2311214 Primary Facility 2014-10-09 Administrative/File Review No Violations Noted On 6/24/14, Cabot Oil and Gas reported a 20 gallon release of hydraulic fluid to the ground due to the failure of a master cylinder of a dump truck. Cabot emailed an electronic copy of their remediation summary report for this spill. Attached is a copy of that report. After a review of the report, the Department concludes that the spill response was adequate and that no further action is required by the Department. VIOIC associated with Inspection Record # 2282584.
2421129 Primary Facility 2015-10-23 Routine/Complete Inspection No Violations Noted The Department was on site to perform a routine complete inspection at the Plonski I Well Pad (ESG13-115-0085). The wells associated with this site are:
2453886 Primary Facility 2016-02-23 Routine/Complete Inspection No Violations Noted On 2/23/16, the Department collected water samples from a nearby complainant. The samples were clear with no effervescence observed. The nearest gas well activity/development to the complainant’s water well are the following 3 Cabot well pads:

Reilly J Pad This is the closest pad and is located approximately 2900’ away. There is only one well on this pad, the 3H which is in production. The well was spud on 8/10/2012, drilling was completed on 10/12/2012, and stimulation was completed on 1/10/2013.

Plonski I Pad This pad is located approximately 3500’ away. There are 5 producing wells on this pad. All the wells were spud in June, 2014 with stimulation completed for all three wells in November, 2014.

Stellitano A Pad This pad is located approximately 4000’ away. There are 8 drilled wells on this pad that are currently being stimulated.

There are no violations associated with this complaint inspection.

The cellar was about a quarter full of water and ice with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 640psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:05p.m.

2618195 Primary Facility 2017-07-24 Routine/Complete Inspection No Violations Noted Today' inspection, 7/20/17 is being conducted as a routine inspection. A work over rig is on-site working. The entire site is stable. Rig appears to have good containment. All slopes stable, no violations noted. No photos attached due to a technical glitch.

The (20x13) annulus was above the ice in the cellar and there was no gas detected. The (13x9) and (9x5) annuli vent pipes were connected together and venting to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was in production. There was water 6' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2800538 Primary Facility 2018-11-06 Routine/Complete Inspection No Violations Noted This inspection was conducted to confirm Cabot's assessment that the project meets the minimum requirements for permit termination. The Department’s determination is that the site meets the requirements for permit termination. I recommend the NOT be approved.
