2309427 | Primary Facility | 2014-09-15 | Incident- Response to Accident or Event | No Violations Noted | Arrived at the site at 12:50 to conduct an inspection related to an estimated (300-400) barrel flowback fluid spill which occurred on 8/9/14. Flowback procedures and natural gas flaring were in progress as the Department arrived to the location. An excavator was in the process of removing the frac tank battery containment liner from the pad surface. The liner was being transferred to a container in preparation for disposal. The primary purpose of the inspection was to evaluate the pad surface beneath the frac tank battery containment due to the spill that occurred on 8/9/14. The pad surface was evaluated in the general vicinity where the spill occurred for evidence of contamination in the in the event of potential compromised liner integrity as the flowback fluid was spilled to containment. Based on visual observations and site evaluations, there is no evidence that the spill on 8/9/14 impacted the surface of the ground on and off the well pad. |

2440317 | Primary Facility | 2016-01-13 | Incident- Response to Accident or Event | No Violations Noted | The area around the pad was stable and appeared to have all BMPs in place. There was vegetation visible on site through the snow cover.
There are no violations. If there are any questions, please feel free to call me at (570) 346-5539. |
2498588 | Primary Facility | 2016-07-01 | Routine/Complete Inspection | No Violations Noted | At time of inspection, arrived on site at 12:35p.m., the cellar was full of water with no bubbling noticed, the (20x13) vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) vent pipe was open and venting into the atmosphere with no gas detected. There was 913psi on the production casing and 455psi on the production tubing. I took a walk around the location and everything looked OK.
I left the site at 12:50p.m. |
2588478 | Primary Facility | 2017-04-27 | Routine/Complete Inspection | No Violations Noted | At time of inspection, arrived on site at 10:50a.m.
The cellar was full of water with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) annulus vent pipe was open and venting into the atmosphere with no gas detected. There was 550psi on the production casing and 367psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:20a.m. |
2697770 | Primary Facility | 2018-02-15 | Routine/Complete Inspection | No Violations Noted | At time of inspection.
The well was in production. The cellar was full of water and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |
2793493 | Primary Facility | 2018-10-22 | Site Restoration | No Violations Noted | I performed a RTNC inspection for for the Stickney well site. The site had its last company E&S inspection on 10/13/2018.
The pad surface area was 180196 square feet. The weather was overcast and 40 degrees. The pad surface and access road were dry. The access road was solid and firm, as was the pad surface. The area was quite rocky. The cut and fill slopes were well vegetated, wherever possible. The rocky nature of the area precludes growth where there is no soil, however wherever there was soil present, there was perennial growth exceeding 70%, as required by the Department. The area was stable and solid and the well was properly labeled. The PCSM infiltration basins were installed and vegetation was adequate on the available soil. There were no violations noted. The well was properly labeled |
2794697 | Primary Facility | 2018-10-25 | Routine/Complete Inspection | No Violations Noted | At time of inspection.
The well was in production. The cellar was full of water and there was no bubbling noticed. There was a methanol tank and pump (see photo) in containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |
