2400587 Primary Facility 2015-08-20 Administrative/File Review No Violations Noted This administrative inspection has been created to document Cabot’s remediation of an approximately (10-15) gallon release of oil based drilling fluids to the pad surface that occurred on 8/31/14 as documented in the Department's 9/3/14 inspection report. Cabot submitted a Remediation Summary Report prepared by Resource Environmental Management Incorporated of Montrose, PA to the Department on 8/4/2015. The report indicates and on-site track-hoe was utilized to remove impacted soils immediately after the release occurred. A review of the Remediation Summary Report and e-mails submitted by Andy Mehalko, Environmental Supervisor of Cabot Oil and Gas, to the Department on 8/6/15 and 8/17/15 indicates that the remediation was adequate. No further action is required of Cabot at this time.
2471219 Primary Facility 2016-03-29 Follow-up Inspection No Violations Noted GDS responded to the release and used rakes and shovels to scrape the access road to the well site. He explained that this was done on the same day that the release occurred. He mentioned that they originally believed that 15 gallons were lost and that 5 impacted the ground however they now re-estimated it to 1 gallon off of containment. Mr. Watson said that it was reported to him that the majority was lost on SR 2024. He also explained that once GDS cleaned the impacted material up, that they stored it in a drum. The excavation was not screened because it was considered a small spill and no samples were collected. Mr. Watson explained that the release occurred around the area where the asphalt/dirt roads interface.
2471251 Primary Facility 2016-03-30 Follow-up Inspection No Violations Noted Steve Watson (Cabot Oil & Gas) contacted me on 3/25/16 at 1019 hours to report a release of (15) gallons of antifreeze from a truck in route to the Makosky Pad. Mr. Watson stated the following. “The release occurred at 0930 hours on 3/25/16. A hose clamp failed on the truck. The release occurred near the lease (access) road entrance. Approximately (10) gallons impacted the ground surface while the remaining (5) gallons impacted the paved road, PA State Route 2024. The well site has since dried from the rain event. The areas that were photographed by Mr. Schoonover (those that appeared to have fluid running over them) now have a white residue on the surface of the ground. The Department collected three separate samples: One within the excavation, one within the white residue, and one on the access road away from the excavation and closer to the access road gate. The soil samples will be analyzed for glycol in response to the antifreeze release.
2475287 Primary Facility 2016-04-15 Administrative/File Review No Violations Noted The Pennsylvania Department of Environmental Protection Bureau of Laboratories in Harrisburg, PA submitted sample results to the Department on 4/7/16 and 4/12/16. The Department collected the samples in the impacted area on the access road on 3/30/16. The reported results (the PaBol is not accredited for ethylene glycol analysis) indicate a concentration of (6,840) mg/kg of ethylene glycol in sample identification number 4559-213, point 001, and a concentration of (2,960) mg/kg of ethylene glycol in sample identification number 4559-214, point 002. These qualitative sample results indicate a potential exceedance of the Act 2, Residential Soil to Groundwater, Statewide Health MSC for Ethylene Glycol of (1,400) mg/kg. The concentration of ethylene glycol detected in the samples indicated the antifreeze spill was not properly remediated at that time.
2476704 Primary Facility 2016-04-20 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:05 a.m., the cellar was about a quarter full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 657psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 12:00 p.m.

2483817 Primary Facility 2016-05-13 Administrative/File Review No Violations Noted On 5/12/16, Andy Mehalko, Environmental Supervisor, of Cabot Oil and Gas provided the Department a Remediation Summary Report prepared by Resource Environmental Management Incorporated of Montrose, PA. The Remediation Summary Report states the following information; Resource on behalf of Cabot mobilized to the site later in the day on 3/30/16 to evaluate soils. They had identified an excavation measuring approximately 7’X3’ x 1-2” on the east side of the access road approximately (83) feet form SR 2024. They collected (3) samples in the same locations as the Department had collected samples on the lease road. Concentrations of ethylene glycol exceeded the Statewide Health Standard (SHS) residential, used-aquifer (R-U) Medium Specific Concentration (MSC) in two (2) of the three (3) samples. Based on these results, additional soil remediation was scheduled by Cabot.

Ethylene Glycol was reported at (6,840) mg/kg in sample number 4559-213 and (2,940) mg/kg in sample number 4559-214, both samples collected

2551809 Primary Facility 2017-01-09 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a routine complete inspection of the Makosky T well site, ESX10-115-0050.

There are 8 wells associated with this site. They are as follows: 115-21586 115-21587 115-21588 115-21589 115-21590 115-21591 115-21592 115-21593 The pad has been considerably shrunk down and it appears PCSMs have been installed. Growth is good, even in recently worked areas. The access road is in good shape with no issues at this time.

There were no erosion or sedimentation issues on site at this time.

2665542 Primary Facility 2017-11-13 Site Restoration No Violations Noted Photo 1 is a picture of infiltration berm # 1. Photo 2 is a picture of the pad layout. This inspection was conducted to confirm Cabot's assessment that the project meets the minimum requirements for permanent stabilization. The Department's determination is that the site meets the requirements for permit termination. I recommend the NOT be approved.
