2537817 Primary Facility 2016-11-17 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:15a.m., the cellar was about a quarter full of water with no bubbling noticed. The (13x9) vent pipe was open and venting into a small poly tank, the (9x5) vent pipe was open and venting into a small poly tank. There was 508psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 11:40a.m.

2575385 Primary Facility 2017-03-24 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection of the Acker Well Site. The E&S Inspection Log indicated that Sam Wiernusz conducted the most recent inspection on 3/13/17.

Operator, please be advised that 102.4(b)(5)(x) states "A maintenance program which provides for the operation and maintenance of BMPs and the inspection of BMPs on a weekly basis and after each stormwater event, including the repair or replacement of BMPs to ensure effective and efficient operation. 2624158 Primary Facility 2017-08-09 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:00p.m.

The cellar had water at the bottom with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 486psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:30p.m.

2670132 Primary Facility 2017-12-12 Routine/Complete Inspection No Violations Noted I inspected the Acker well site on December 12, 2017. The access road was excellent. It was stable and the front part was paved and had no potholes. The access road runs next to a valve set. This was camouflaged from homeowner view by evergreens, which were healthy and lush. The site itself was small and well maintained. The pad surface was solid and stable and the cut and fill slopes were gentle and well vegetated. There were no erosional issues on site at the time of inspection.

There were no violations.

2732601 Primary Facility 2018-05-16 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 5' below grade in the cellar and there was no bubbling noticed, there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes are connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2832299 Primary Facility 2019-01-28 Routine/Complete Inspection No Violations Noted At time of inspection.
