2437796 Primary Facility 2016-01-05 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:00 p.m., the cellar was full of water with ice on the top, the (9x5) vent pipe was open and venting into the atmosphere with gas detected at 100% LEL and 16% methane, there was 954psi on the production casing and 949psi on the production tubing, I took a walk around the location and everything looked OK.

I left the site at 12:15 p.m.

2451953 Primary Facility 2016-02-08 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete inspection of the Butler Well Pad (ESG13-115-0089). The well associated with this site is as follows:

115-21476.

The site was well vegetated and there were no eroional issues apparent at this time.

2472032 Primary Facility 2016-03-16 Routine/Partial Inspection No Violations Noted The Department was on site to conduct a Routine Partial Inspection of the Butler Well Site located in Apolacon Township, Susquehanna County. At the time of the inspection, it appeared as though the well site met temporary stabilization requirements. Steve Haight signed off for an inspection on 3/15/16. There was a small area of stained rock on the access road close to the well pad. The Department requests that the operator investigate this issue, within 2 days of receipt of this report, and contact the Department with your findings.
2543578 Primary Facility 2016-12-07 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:00a.m., the cellar was about a quarter full of water with no bubbling noticed, there was cement between the (20x13) and (13x9) annuli’s with no gas detected, the (9x5) vent pipe was open and venting into the atmosphere with gas detected at 100% LEL and peaked at 8% methane with no measurable flow, there was 660psi on the production casing and 354psi on the production line. Phillip Kithcart was pumping water through the production line to clear salt in the line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 10:40a.m.

2628874 Primary Facility 2017-08-24 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. I detected gas at the (9x5) annulus, it peaked at 84% LEL and 6% methane. There was a flat bed trailer with chemical containers on it that was on containment. The containment had two holes that I noticed, l called Kriss Perritt of Repsol to let him know of the holes. There was no indications of the containers leaking.

2735717 Primary Facility 2018-05-29 Routine/Complete Inspection No Violations Noted Jon Sassi and I inspected the Butler well pad for a RTNC inspection. The wells associated with this pad are as follows: 115-21476. The cut and fill slopes were well vegetated. There was a stockpile of stone to repair the rce on hand near the entrance. The access road and pad surface were both solid and stable. There were no noted erosional issues at the time of inspection.

There were no violations observed.

2739241 Primary Facility 2018-06-08 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 3.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. There was cement between the (20x13) and (13x9) annuli and there was no gas detected. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2835128 Primary Facility 2019-02-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of snow and ice, I was unable to check for gas. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
