Well permits that have been issued for this pad are the 3H (115-21468), 5H (115-21458), 3H (115-21470) and the 6H (115-21471). All perimeter BMP’s in place and include red lined additions. The pad is 100% complete. The tank pad is approximately 50% complete. No violations noted. Street address is 1652 Toad Holler Road.

2464472 Primary Facility 2016-03-14 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:30 p.m., the cellar was full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 573psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 14:00 p.m.

2545874 Primary Facility 2016-12-16 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:00a.m., the cellar was about three quarters full of ice and snow. The (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 557psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 10:35a.m.

2633612 Primary Facility 2017-09-07 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. All the annuli vent pipes were venting to the same tank. I took a walk around the location and there were no obvious concerns noticed.

2728163 Primary Facility 2018-05-08 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Bennett pad. The wells associated with this site are as follows: 115-21468, 115-21470, 115-21471. The wells were properly labeled.

The access road and pad surface were solid and stable.

The cut and fill slopes were well vegetated, with quite a lot of green showing. There were no areas of instability noted.

There were no violations noted.

2742566 Primary Facility 2018-06-18 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 3' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (20x13) and (13x9) annuli vent pipes were connected together and venting to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2836938 Primary Facility 2019-02-07 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 2/5/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
