Minichi 9 was tripping out of hole on the intermediate string, expected to run casing and pump cement later today. Surface string went well. There were no water shows, 23 barrels of good cement to surface. The floats held and the rig waited 8 hrs before any casing disturbance
2355449 Primary Facility 2015-03-16 Routine/Complete Inspection No Violations Noted Minichi Drilling Rig #9 is on the location currently in the process of drilling the Longacre (Range Unit #43) 5H well at a depth of (1,620) feet. I noticed a rainbow colored petroleum sheen in water on the pad surface where I parked my truck near the production containment and trailers. I pointed out the sheen to Mr. Ruddell upon exiting the Company Man Trailer in preparation to survey the site. Mr. Ruddell order on-site employees to immediately begin the process of removing the sheen in the water via a vacuum truck. I also noticed dark colored fluid on the pad surface. Mr. Ruddell informed me that hammer oil has dripped off the ends of four and a half inch drill pipe to the ground surface of the pad. The drill pipe was not staged on secondary containment. Mr. Ruddell stated that the area of the pad surface impacted from the hammer oil dripping from the drill pipe will be cleaned up immediately with shovels. I noticed the petroleum sheen was no longer visible in the water as I was about to get into my truck and leave the site.

2557661 Primary Facility 2017-01-03 Site Restoration No Violations Noted The Department (Steve Brokenshire and I) was on site at the RU#43-Longacre well site located at 767 Longacres Road Susquehanna, PA to conduct a Restoration Inspection in response to the submittal of a Notice of Termination (NOT). The E&S Inspection Checklist indicated that SRT performed a weekly inspection on 11/17/2016 and that no attention was needed. A note on the E&S Plan stated that the wash rack was removed on 6/7/2016.

The Department noted that Appendix A of the NOT submission indicated that a Subsurface Infiltration Bed was installed on the site as the PCSM BMP. The Department's review of the E&S Plan revealed this PCSM BMP was not included in the ESCGP. The Department contacted SWN and spoke with Justin Thomas who explained that SWN believes that it was included in error. The Department found that this permit was disposed on 10/19/2011 therefore it had expired prior to the NOT receipt date of 10/26/2016.

At this time the Department requests that the operator contact the DEP Northcentral Reg

2585072 Primary Facility 2017-04-18 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:35p.m.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 670psi on the production casing and 672psi on the production line. The location is being used as a staging area for the sand trucks for the frac job at the Gaylord pad. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:15p.m.

2585477 Primary Facility 2017-04-21 Routine/Complete Inspection No Violations Noted Todays inspection, 4/18/17 is a routine inspection. The well pad appears to be being utilized as an overflow site for equipment related to the stimulation activities across the street on the Gaylord Pad. Some standing water on pad. Highest conductivity reading observed was 432 micro Siemens. Well site has good stability. No violations observed.
2690142 Primary Facility 2018-02-01 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below ice and was unable to check for gas. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2791328 Primary Facility 2018-10-18 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2855249 Primary Facility 2019-03-12 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
