2234488 Primary Facility 2014-01-06 Routine/Complete Inspection No Violations Noted New Park Mats and Integrated Services were in the processing of breaking down rig mats used for secondary containment. Some of the rig mats were in the process of being pressure washed/steam cleaned. A vacuum truck labeled fresh water was removing water on containment as the mats were being washed. This process took place on a separate secondary containment system from the containment system use for well stimulation procedures. There were some areas of compromised liner integrity observed at the well site. I noticed petroleum like sheens, rainbow in color, on containment relatively close to the recycled water tanks. Mr. Pirner stated the sheens will be removed from containment. Conductivity readings were in the range of (2-4) milliSiemens/cm in standing water on containment between the above ground storage tank used to store freshwater needed for well stimulation procedures and the recycled water tanks needed for the same purpose.

I took a walk around the location and there were no obvious concerns noticed.

I left the site at 15:45p.m.

At the time of my arrival I met with Sam Wiernusz (TEC) who was on site conducting a Post Precipitation Inspection. He joined me for the inspection.

The cellar had water at the bottom with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 618psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:45p.m.

2730655 Primary Facility 2018-05-16 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water in the cellar 5' below grade and there was no bubbling noticed, there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2831048 Primary Facility 2019-01-24 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water, snow and ice 4' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
