2561856 Primary Facility 2017-02-08 Incident- Response to Accident or Event No Violations Noted The Department (Jon Sassi-DEP, EPS) received a phone call from Mike Demming (Senior Regulator Analyst-SWN) on 2/7/17 at 1601 hours to self report a release of less than 5 gallons of diesel fuel. The release was discovered at 1405 hours. Mr. Demming explained that a US Well Services heater trailer (line heater which keeps water from freezing during hydraulic fracturing operations and has since been taken to their yard for repair) was exiting the location and was tilting forward on the access road when the diesel fuel overflowed the belly pan and was released to the access road. The diesel fuel traveled along the access road being carried by stormwater (raining at the time of the release) downslope and flowing into the roadside ditch along Hawk Road. The belly pan is used to collect leaks and overflow from the heater trailer. The operator failed to clean the belly pan prior to demobilization. A mini excavator will be used to remove any impacted material and a vac truck will remove and impacted fluid. S
2571141 Primary Facility 2017-03-09 Follow-up Inspection No Violations Noted We met with Mike Deming (Senior Regulatory Analyst-SWN) and Adam Brace (Production Foreman-SWN) during this inspection. During the inspection, the Department was approached by a complainant who voiced their issues concerning the condition of Hawk Road. They explained that the increased truck traffic due to activity at the well site has pushed material into the roadside ditch (because the trucks are driving at the edge of the roadway)causing sediment laden water to enter the stream.

They requested that the Department have SWN repair Hawk Road and install catchment basins at the culvert outlets along the roadway. The complainant explained that when Alta was operating the site, that a similar complaint was made due to sediment entering a pond in the vicinity. They showed us an area where they said rock was added to the roadside ditch in response to that issue.
