2467831 Primary Facility 2016-03-16 Routine/Partial Inspection No Violations Noted The Department was on site to conduct a Routine Partial Inspection of the Repine Well Site located in Apolacon Township, Susquehanna County. At the time of the inspection, it appeared as though the well site met temporary stabilization requirements.
2500984 Primary Facility 2016-07-13 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:45a.m., the cellar was about half full of water with no bubbling noticed, the (9x5) vent pipe was open and venting into the atmosphere with no gas detected. There was 801psi on the production casing and 410psi on the production tubing. I took a walk around the location and everything looked OK.

I left the site at 11:20a.m.

2534678 Primary Facility 2016-11-01 Routine/Complete Inspection No Violations Noted The Department was on site on 2/1/2016 to conduct a Routine Complete Inspection of Talisman's Repine Well Pad (ESG13-115-0059). The wells associated with this site are:

The well pad was stable, but due to recent heavy rainfall, the area covered by a bermed liner surrounding the wellheads had about 4 inches of water covering the surface. There was no bubbling from the well cellars apparent, and the water was contained. It would be recommended to vacuum the water from this area to prevent any potential problems. There was adequate vegetation throughout the site and there were no erosional issues apparent at this time.

2588140 Primary Facility 2017-04-13 Routine/Complete Inspection No Violations Noted Arrived at the site at 11:35. The weather conditions were partly sunny with temperatures in the upper 40's. Repsol submitted a notification to the Department in Greenport (esubmissions) on 4/11/17 at 4:10 PM for Notice prior to installation of a modular storage structure per 78.56(a)(4) (unconventional wells). The notice indicated the modular storage structure (DYNA tank) construction/ installation would commence on 4/11/17. The following comments were generated upon submission of the notification. " This proposed date does not meet the regulation’s 3 business day advance notice requirement. See the date provided for the question: "* Date and time construction/installation is to begin" If you wish to send this notification to DEP anyway, you may use the Submit button at the bottom of this page to confirm this alert and send. I was accompanied by Frank Hoehle, DEP O&G WQS of the Scranton District Office. We met with Shaun Smith, site company man of Repsol. Mr. Smith stated the following in reference to sit
2590058 Primary Facility 2017-05-02 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:00a.m.

The cellar was full of water with no bubbling noticed. The (9x5) annulus vent pipe was open and venting into the atmosphere with no gas detected. There was 824psi on the production casing and 385psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:40a.m.

2590541 Primary Facility 2017-05-02 Routine/Complete Inspection No Violations Noted I arrived at the site at 11:00. The weather conditions were partly cloudy skies and breezy with temperatures in the 50's. I was accompanied by David Schoonover, DEP Oil and Gas Inspector of the Scranton District Office. We met with Shaun Smith, completions consultant, and Kris Perritt, Environmental Specialist, both of Repsol. Mr. Smith and Mr. Perritt stated the following. "The hydrostatic testing of the DYNA tank started on 4/13/17 and ended on 4/15/17. The test was a one time success. The tank was filled a week later with production and flowback fluid from other Repsol pads. The flowback fluid was transferred here from our Kropp Pad which is nearby. The fluid level in the tank is currently at (30) feet. It will be filled by tomorrow to a depth of (33-33.5) feet. We have been removing rainwater inside the DYNA tank containment and transferring this water to the frac tanks. We take daily conductivity readings of water on and off containment at the site. 2620116 Primary Facility 2017-07-18 Routine/Complete Inspection No Violations Noted I arrived at the site at 11:33. I was accompanied by Frank Hoehle, PADEP O&G WQS during the inspection. The weather conditions were partly cloudy skies with a light breeze, very warm and humid with temperatures in the low to mid 80's. Repsol submitted a stimulation notification to the Department on 7/6/17. The notification indicated that stimulation (hydraulic fracturing) of the wells would commence on 7/12/17 at 6 AM. We met with Dave Coxhead, site safety man of REM Security and Dell Gunn, site company man and completions consultant of Repsol. Mr. Gunn and Mr. Coxhead stated the following in reference to site activities. "Frac operations commenced on 7/14/17. The 1H well is in the process of being stimulated in the 5th of the 36th stage. Zipper frac operations are in progress. The 1H, 4H, 5H and 7H wells will be stimulated. The 2H and 3H wells were in production, but are currently shut in". The Department left the site at 12:41.

2686313 Primary Facility 2018-01-23 Routine/Complete Inspection No Violations Noted I inspected the Repine T well site on Jan 23, 2018. The site access road was paved and in good shape. The site surface had a lot of standing water due to recent heavy rains and snowmelt. Despite the water, the pad surface was solid. The slopes around the site were well vegetated and stable. The wells were all properly labeled. The area where a large water tank had been (now removed) was well graded and had no issues. There were no erosional issues noted.

2710189 Primary Facility 2018-03-22 Routine/Complete Inspection No Violations Noted I performed a Routine Complete inspection of the Repine well pad. Work was being performed involving the sand separators on the well site. The Access road was paved and plowed, and the pad surface was solid and stable. The fill slope and area surrounding the pad and access road were all vegetated, visible through the snow cover. There were no erosional issues observed. The wells were properly labeled.

2752793 Primary Facility 2018-07-17 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 1.5' below grade in the cellar and there was no bubbling noticed. There was a 500bbl tank on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. 2760838 Primary Facility 2018-08-06 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Repine well site. The wells associated with this site are as follows: 115-21351, 115-21352, 115-22146, 115-22147, 115-22148, 115-22149.

The wells were properly labeled. (Photo 1). The well cellars were full of water due to recent rains. None appeared to be bubbling at the time of inspection.

The access road was paved and the macadam was in good shape. There was no erosional issues observed along the access road ditch.

The cut slope and topsoil pile were stable and well vegetated. (Photo 2). The fill slope was stable and well vegetated also. (Photo 3). The grasses were perennial. There was no slumping or erosional issues observed at the time of inspection.

The pad surface was well graded and stable.

There were no violations noted.

2763191 Primary Facility 2018-08-13 Routine/Complete Inspection No Violations Noted While on site for a reported release I performed a perfunctory e & s inspection, as I was recently at the site. There were no erosional issues noted, and all slopes appeared stable despite the monsoon-like rains the area was experiencing.

The wells were properly labeled.

The well was in production. There was water and ice .5' below grade in the cellar and there was no bubbling noticed. The (9x5) annulus was venting to the atmosphere, see well information for gas show. There was a 500bbl tank about 3/4 full of fluid on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
