Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2155703 Primary Facility 2013-04-04 Routine/Partial Inspection No Violations Noted
2173928 Primary Facility 2013-06-03 Drilling/Alteration No Violations Noted Conductor set
2464415 Primary Facility 2016-03-03 Routine/Complete Inspection No Violations Noted On 3/3/16, the Department conducted an inspection of the SWN RU54 Tingley Pad while primarily conducting an inspection of the DTE/Susquehanna Gathering 53-54 Well Connect. The pad appears to have built on an old bluestone quarry. Pad is primarily stone stabilized. DTE’s encroachment on the pad LOD is primarily through the stone stabilization. DTE has installed new controls which have been integrated into the well pads controls. Overall, E&S controls look good. All 3 wells associated with this pad are in production. No violations noted.
2466620 Primary Facility 2016-03-18 Routine/Complete Inspection No Violations Noted

2561224 Primary Facility 2017-02-13 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 15:10p.m., the cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 655psi on the production casing and 529psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 15:50p.m.

2654066 Primary Facility 2017-10-30 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2755362 Primary Facility 2018-07-24 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The spay on liner on the inside of the containment for the production fluid tank was showing weather deterioration above the water level. I contacted SWN to have them check it out and have it repaired if needed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2775633 Primary Facility 2018-09-13 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Site has very good permanent stabilization. No E&S issues noted. No violations noted.
2855747 Primary Facility 2019-03-19 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar with a MSA Altair 5X multi gas detector that was calibrated on 3/4/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
