The (13x9) vent pipe was open and venting into the production tank, the (9x5) vent pipe was open and venting into the production tank. There was 491psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 12:25p.m.

2561072 Primary Facility 2017-02-13 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection of the Castrogiovanni well site. At the time of the Department's arrival, a well tender was on site. I reviewed the E&S Inspection Log which showed that Sam Wiernusz conducted the most recent inspection which was listed as a Post Precipitation Inspection on 2/2/17. The well site was mostly snow covered. No obvious issues were noted.

The cellar had water at the bottom with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 503psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:15p.m.

The E&S Inspection Log stated that Scott Rockwell (TEC) conducted the most recent inspection on 8/16/17. Currently the 1H, 5H, 4H, and 8H wells are present however the Department was unable to locate well tags. The Department contacted Steve Watson concerning this issue and requested a return call.

The well was in production. There was water 2' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2776784 Primary Facility 2018-09-18 Routine/Complete Inspection No Violations Noted I performed a Routine inspection of the Castrogiovanni well site. It was sunny, following some heavy rains.

The site was compact (104,999 sq. ft.). The access road and well pad surface were stable and solid. The cut slope was well vegetated, above 70% perennial vegetation, except for a small section where the ground was too rocky to provide for heavy vegetative cover. The fill slope was well vegetated and the surrounding area was involved in active agriculture (hayfield, corn). There was a row of evergreen trees planted along the fill slope, and all looked healthy.

The entire area was solid and stable with good vegetative cover.

There were no violations noted.

2833561 Primary Facility 2019-01-30 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was snow and ice 2' below grade in the cellar and there was no gas detected. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
