Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2167827 Primary Facility 2013-05-07 Routine/Partial Inspection No Violations Noted -The roadside ditches have not been lined and they have not been shaped as per design.
2167938 Primary Facility 2013-05-08 Follow-up Inspection No Violations Noted
2167981 Primary Facility 2013-05-02 Routine/Partial Inspection No Violations Noted
2179934 Primary Facility 2013-06-12 Drilling/Alteration No Violations Noted Spudder rig on location
2489086 Primary Facility 2016-05-31 Site Restoration No Violations Noted Inspection of the Carpenetti P1 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. The restored pad size is consistent with the PCSM/Site Restoration Plan. Post construction storm water management devices were in place according to approved plans and appear to be functioning properly.

Based on the site inspection, I recommend this notice of termination be APPROVED.

2639143 Primary Facility 2017-08-09 Routine/Complete Inspection No Violations Noted Today's inspection, 8/9/17 was conducted as a routine inspection. 4 wells have been drilled and all appear to be in production. Very good stability exists at this site. Perennial vegetation is well established. No violations noted.
2732843 Primary Facility 2018-05-18 Preoperation Inspection No Violations Noted The Department (Casy Baldwin and I) was on site to conduct a Preoperational Inspection in accordance with 3258(a.1). Upon arrival the Department reviewed the E&S Inspection Log which indicated that the most recent inspection was conducted on 5/17/18. The Department met with Mike Murnin (Construction Foreman-Cabot ) and Larry Cheatwood (Company Man-Cabot). This site was previously permitted under ESCGP ESX12-115-0191. The previous ESCGP was Terminated in May of 2016 and was re-permitted under the new ESCGP2 to enlarge the pad and drill an additional 9 wells. At the time of the inspection they were placing shot rock on the site. On the eastern side of the access road where it meets the pad they redlined a ditch on the fill and will grade it back and allow stormwater to travel to the previously existing access road roadside ditch.
