2726802 Primary Facility 2018-05-02 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Bishop well pad. The wells associated with this pad are as follows: 115-21125, 115-21185, 115-21186, 115-21187, 115-21188, 115-21189, 115-21311. The wells were properly labeled.

The access road and pad surface were stable.

The cut and fill slopes were stable and well vegetated and the pad surface was quite small. There were no erosional issues apparent during the inspection.

There were no violations.

2800793 Primary Facility 2018-11-07 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection on the Bishop well site. The weather was mostly sunny and 48 deg. F. There was previously heavy rain.
The access road was solid and stable and the area to the sides was well vegetated and stable. The well pad surface was puddled due to recent rains but had no erosional issues occurring at the time of inspection.
The slopes and the area surrounding the site were well established with perennial grasses. The growth exceeded the 70% vegetative standard required by the Department. There was no evidence of slumping or slope failure at the time of inspection.
All temporary BMPs have been removed from the site and there were no indications of any adverse affects due to their removal.
The permanent PCSMs were all installed according to plan and appeared to be functioning as intended.
The area as a whole was solid, stable and had good growth, with no temporary BMPs remaining and all permanent PCSMs in place. The well pad area was compact and pulled in as m
