Crews were finishing up the final application of stone to the access road and final grading on the topsoil piles. Perimeter BMPs have been installed and appear to be functioning as intended. The following BMPs were observed; Wood Chip FIlter Berms, Rock Construction Entrance with Tire Wash Rack, Slope Stabilization, Drainage Swales/Road Side Ditches. Stabilized access road and well pad and Temporary stabilization. We discussed ensuring that all ditches and swales are constructed to the proper final dimensions and ensuring that all wood chip filter berms are installed on-contour.

2276354 Primary Facility 2014-05-22 Routine/Complete Inspection No Violations Noted Falcon Rig# 25 is the active drilling rig on the location air drilling the Mitchell South 4H well. Bud Woolsey, site company man and consultant of SWN accompanied me during the inspection. The current drilling depth is (687) feet. The Mitchell North 3H and Mitchell South 6H wells are in the production phase of operations. Areas surveyed during the inspection included the drilling rig and associated equipment containment systems, backyard, ground surface of the Pad and production containment system and equipment. The drilling secondary containment systems consist of composite rig mats and Western liner. The Western liner is a (40) mil liner. There is an overlying and underlying layer of felt in the liner. The containment perimeter berm is made of railroad ties. The containment systems at the site appeared to be in good condition.

The vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 651psi on the production casing and 379psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 12:05p.m.

2753246 Primary Facility 2018-07-18 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The spay on liner on the inside of the containment for the production fluid tank was showing weather deterioration above the water level. I contacted SWN to have them check it out and have it repaired if needed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2822347 Primary Facility 2019-01-04 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Mitchell well pad. The weather at the time of inspection was partly sunny and about 35 degrees F.

The E&S log was up to date and the site sign was legible and in good shape.

The access road was a bit muddy but was stable. There was evidence of recent truck traffic onsite but no rutting or erosional issues were present.

The pad surface was damp but solid. There were no ruts or erosional issues apparent. The wells on site were properly labeled. A crew was on site performing a hay blowing operation. There were new permanent PCSMs recently constructed.

The cut slope was green, stable and well vegetated with perennial vegetation and the fill slope was not as well vegetated but was stable. The fill slope was extremely rocky in the areas of low vegetation but where there was enough soil present, the vegetation was adequate.

The site was stable and there were no noted violations at the time of inspections.

2822737 Primary Facility 2019-01-03 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. A crew was on-site upgrading the 2 sediment basins to their bioretention design. Overall the site has good Stabilization. No E&S issues identified. No violations noted.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar with a MSA Altair 5X multi gas detector that was calibrated on 3/4/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
