2715956 Primary Facility 2018-04-06 Site Restoration No Violations Noted This inspection was conducted to confirm Cabot's assessment that the project meets the minimum requirements for permit termination. Photo 2 shows the only PCSM control designated for this site, 6.49 acre Wooded Filter Strip. The Department’s determination is that the site meets the requirements for permit termination. I recommend the NOT be approved.
2748523 Primary Facility 2018-06-15 Routine/Complete Inspection No Violations Noted The Department arrived on site at 1415 hrs to conduct a RTNC inspection. Present to represent the Department were Gene Rickard (WQS-DEP) and myself.

The access road, pad surface, and cut/fill slopes were solid and stable. Required BMPs were present and in good condition.

The well heads were clearly and properly labeled with names and permit numbers. Required signage was present at entrance to access road. All required paperwork was present in the mailbox.

It should be noted that while screening the water in the produced fluid tank's containment, the conductivity meter indicated the water had extremely high conductivity, over the range of the meter. (Photo 6) The operator should address this issue promptly and inspect the produced fluid tank to make sure its integrity has not been compromised.

The Department concluded its investigation and left the site at 1445 hrs. No violations were noted.
