Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2125303 Primary Facility 2012-12-12 Drilling/Alteration No Violations Noted In process of rigging up Patterson 263. Well has been spud. Secondary containment in place on pad under rig and around mud storage tanks. No violations observed at this time.
2151292 Primary Facility 2013-03-11 Drilling/Alteration No Violations Noted During well flaring on March 11, 2013, Cabot discovered the release of an unknown volume of brine to soil that apparently occurred during previous hydrofracturing operations. ECB received a final report from Cabot's consultants documenting their cleanup on June 10, 2013. Act 2 relief from liability for this incident was granted on June 25, 2013.
2163169 Primary Facility 2013-03-11 Routine/Complete Inspection Violation(s) Noted
2514895 Primary Facility 2016-08-26 Routine/Complete Inspection No Violations Noted
2585043 Primary Facility 2017-04-20 Routine/Complete Inspection No Violations Noted Todays inspection, 4/3/17 is a routine complete inspection. The entire site has good stability. Silt sock present could probably be removed. There is only one well, the 6H drilled and in production. No violations noted.
2665776 Primary Facility 2017-11-30 Routine/Complete Inspection No Violations Noted Arrived on location at: 11:30. Weather: Clear, mild ~ 42 degs. The 6H well was producing and is the only well on pad. This wells (13x9) and (9x5) annuli were open and venting, plumbed to a common poly vent tank. A Gas check performed at the common poly vent tank showed 0 % LEL, 0 % CH4. -This wells Prod. casing, Static and Differential pressures were being monitored by a wireless sending unit to onsite Total Flow Control Unit "TFCU". - TFCU readings were taken off control panel at ~ 12:00. (CP= prod. casing press., SP= static press., DP= differential press.)

- 6H: CP= 558.9 psi, SP= 563.2 psi., DP= 113.4 psi. - Overall, well and pad were found in good condition. No obvious concerns noted at time of inspection. No violations noted. Left location at 12:30.

2672769 Primary Facility 2017-11-29 Complaint Inspection No Violations Noted Today's inspection is being conducted as part of an investigation into a nearby water supply complaint. Only one well, the 6H has been drilled and is in production. No violations were identified during this inspection. Good pad stabilization. Good perennial vegetation present.
2818984 Primary Facility 2018-12-18 Routine/Complete Inspection No Violations Noted Today's inspection has been conducted as a routine inspection. Entire site has been reworked. No E&S issues were evident. No violations noted.
2843127 Primary Facility 2019-02-21 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. As the photo's show, the site is snow covered. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
