The E&S Inspection Log indicated that Will Slote conducted an inspection on 12/30/14 and that the site was ok and snow covered at the time. At the time of the inspection, the site was mostly snow covered. No obvious E&S issues were found.

2509073 Primary Facility 2016-08-09 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m., the well was given inactive status on 12/7/15. The cellar was full of water with no bubbling noticed; the vent pipe was open and set up with a Taylor plug to check for pressure, there was a cap on top of the well. There were no lines going to or from the well and there were no tanks or production units on the site. I took a walk around the location and everything looked OK.

I left the site at 13:05p.m.

2602054 Primary Facility 2017-06-07 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:45a.m.

The well was drilled to TD at 11,858' but wasn’t stimulated and granted inactive status on 12/7/15. The cellar was full of water with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was a TA cap on top of the well with a Taylor plug to check for pressure with a Crystal pressure gauge. There are no lines going to or from the well and there are no tanks or production units on site. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:10a.m.

2607889 Primary Facility 2017-06-27 Routine/Complete Inspection No Violations Noted I inspected this site on June 1, 2017. The site was generally stable, although there are weak spots of vegetation, especially along the access road. (See photos). One area in particular along the access road is recommended to be reseeded and stabilized as soon as possible, as the vegetation is gone and the ground is in danger of eroding. The pad surface was stable and solid, with the area around the well heads appearing to have been cemented. The cut and fill slopes were generally in good shape, although the vegetation on the cut slope was struggling a bit due to rocky soil conditions. The access road itself seemed solid and stable.

There were no violations observed during this inspection.

No pressure; well was never perf'd. The operator will cap both CIBPs w/ CMT, gel uphole, and set cmt plugs across intermediate an surface csg shoes (inside production csg). CBLs indicate production casing unrecoverable. Walked the location with the Companyman. There were no issues observed.
