2343416 Primary Facility 2014-12-30 Routine/Partial Inspection No Violations Noted The Department was on site at the Thomas Conaty well site located at 600 Arrowhead Cottage Road Brackney, PA to conduct a Routine Partial Inspection. The E&S Inspection Log indicated that Will Slote conducted an inspection on 12/30/14 and that the site was ok and snow covered at the time. At the time of the inspection, the site was mostly snow covered. No obvious E&S issues were found.
2607891 Primary Facility 2017-06-27 Routine/Complete Inspection No Violations Noted I inspected this site on June 1, 2017. The site was generally stable, although there are weak spots of vegetation, especially along the access road. (See photos). One area in particular along the access road is recommended to be reseeded and stabilized as soon as possible, as the vegetation is gone and the ground is in danger of eroding. The pad surface was stable and solid, with the area around the well heads appearing to have been cemented. The cut and fill slopes were generally in good shape, although the vegetation on the cut slope was struggling a bit due to rocky soil conditions. The access road itself seemed solid and stable.

There were no violations observed during this inspection.
