2505610 Primary Facility 2016-07-26 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:05a.m., the cellar was full of gravel and no gas was detected. The vent pipe was open and set up with a Taylor plug; there was 569psi on the production casing and 567psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 9:30a.m

2598835 Primary Facility 2017-05-25 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 15:00p.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 333psi on the production casing and 277psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 15:25p.m.

2661560 Primary Facility 2017-11-13 Routine/Complete Inspection No Violations Noted I inspected the O'Reilly well pad on November 13, 2017. The well pad was solid and stable, as was the access road. The cut and fill slopes were stable and well vegetated also. There were no erosional issues apparent at the time of inspection.

2704117 Primary Facility 2018-03-12 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and was unable check for gas at the perimeter of the cellar because the gravel was frozen. There was no gas detected at the perimeter of the cellar during a routine inspection conducted on 5/25/17. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2803070 Primary Facility 2018-11-14 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2822989 Primary Facility 2019-01-04 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the O'Reilly well pad. The weather at the time of inspection was about 35 degrees F and partly sunny.

The access road was short and solid. The site sign was legible and accurate.

The pad surface was a bit grown in but solid and stable. There was recently some earthwork performed installing permanent PCSMs on site and all the worked areas were covered with mulch stabilization. There was a small repair in the side of the fill slope but the rest of the fill slope was well vegetated and the whole area was stable. The repaired area was well mulched and stable.

The cut slope was well vegetated and stable as well.

The well sign was legible.

There were no erosional issues apparent and there were no violations at the time of inspection.
