2245206 Primary Facility 2014-02-04 Incident- Response to Accident or Event Pending Arrived at the site at 11:50 in response to a production fluid spill estimated to be between (21-31.50) gallons. Brody Webster of Cabot Oil and Gas contacted PADEP Northeast Region Emergency Response Team on 1/28/14 at 19:37 to report the incident. A brief description of the incident is listed in the emergency response form. The incident states that a one-half to three quarters of a barrel of production water spilled at the Petersen Pad. The spill was contained to the Pad. In a phone conversation with Brody Webster of Cabot Oil on Gas on 2/3/14 he stated that the result of the spill is similar to the causes of recent production fluid spills at other Cabot Well Pads. These spills have been occurring mainly due to production and dump lines freezing.
2267046 Primary Facility 2014-05-01 Administrative/File Review Viol(s) Noted & Immediately Corrected This administrative inspection has been created to document Cabot’s remediation of the small spill which occurred on the H Petersen Pad documented in the H Petersen 1H 2-4-14 inspection report; inspection record # 2245206. A review of the remediation summary report prepared by Resource Environmental and submitted Cabot indicates that the remediation was adequate. No further action is required of Cabot at this time. Attached is a copy of the remediation summary report, the waste manifest associated with this spill, and the 2/4/14 inspection.

2505928 Primary Facility 2016-07-22 Incident- Response to Accident or Event No Violations Noted Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas, contacted me at 8:36 AM on 7/22/16 to report a spill of (200) gallons of a mixture of recycled water and freshwater on the well pad off containment. Mr. Watson stated the following information. “The spill occurred in the Southeast corner of the well pad. Keane was in the process of fracking the 3H well when the spill occurred. A mixture of (25%) recycled water and (75%) freshwater overtopped a (55) gallon drum as part of an air vent tied to a transfer line. The line runs from the tank pad to the well pad. A blue disc made of plastic got sucked up into the fluid hose inside the drum. The disc got stuck above the ball valve in the air vent valve as water was flowing in the transfer line. This caused the water to flow into the hose and out of an opening on the top of the drum, onto the ground. The water flowed out of the drum and onto the ground for 4 or 5 minutes until it was discovered around 7AM. The valve on the air vent
