2477157 Primary Facility 2016-04-21 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:35 p.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 688psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 1:15 p.m.

2748185 Primary Facility 2018-07-02 Routine/Complete Inspection No Violations Noted This inspection was conducted to confirm Cabot's assessment that the project meets the minimum requirements for permit termination. The Department’s determination is that the site meets the requirements for permit termination. I recommend the NOT be approved.
2852399 Primary Facility 2019-03-12 Routine/Complete Inspection No Violations Noted I performed an inspection of the Plonski pad. The weather was overcast and about 32 degrees.

The access road was clear of snow and was just slightly wet, without ruts or runoff problems. The fill slope was visible from the access road and was well vegetated and appeared stable. No rills or ruts were visible.

The pad surface was partially plowed and was solid.

The cut slope was well vegetated and stable.

The site was stable overall and no erosional problems were apparent at the time of inspection.

The wells were all clearly labeled.
