Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2110107 Primary Facility 2012-10-10 Drilling/Alteration No Violations Noted Patterson 538. Co. Man Perk McDowell. Running in hole to start drilling lateral. Drillers log on site and up to date. Proposed TD = 12,700'. Drilling this well and then moving off loction. Walked site. Secondary containment in place and in good condition. Good cement returns reported on both 13 3/8" and 9 5/8" casing strings. Currently at depth of 8157'..drilling on SOBM. no violations observed at this time.
2284287 Primary Facility 2014-06-17 Routine/Complete Inspection No Violations Noted Well in production. Cellar filled with water, no visible bubbling. 50 psi on production tubing. 2 tanks, 1 treater, sand trap
2453913 Primary Facility 2016-02-23 Routine/Complete Inspection No Violations Noted On 2/23/16, the Department collected water samples from a nearby complainant. The samples were clear with no effervescence observed. The nearest gas well activity/development to the complainant’s water well are the following 3 Cabot well pads:

Reilly J Pad This is the closest pad and is located approximately 2900’ away. There is only one well on this pad, the 3H which is in production. The well was spud on 8/10/2012, drilling was completed on 10/12/2012, and stimulation was completed on 1/10/2013.

Plonski I Pad This pad is located approximately 3500’ away. There are 5 producing wells on this pad. All the wells were spud in June, 2014 with stimulation completed for all three wells in November, 2014.

Stellitano A Pad This pad is located approximately 4000’ away. There are 8 drilled wells on this pad that are currently being stimulated.

There are no violations associated with this complaint inspection.

2477067 Primary Facility 2016-04-21 Routine/Complete Inspection No Violations Noted
2552368 Primary Facility 2017-01-12 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:05p.m., the cellar was full of water with a steady bubbling noticed. I checked the bubbling with an Altair 5X gas meter, I got a reading of 100% LEL and peaked at 47% methane. The (13x9) vent pipe was open and venting into the production tank, the (9x5) vent pipe was open and venting into the production tank. There was 747psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. While conducting my inspection, the well tender Chris Chamberling arrived on site. I showed him the bubbling and he called for a water truck to vac the water level down to determine where the bubbling was coming from. A GDS water truck vacced the water level down and it was determined that the bubbling was coming from a lock down connection on the production casing section of the well head. A phone call was made to have it repaired. I left the site at 14:40p.m.
2617282 Primary Facility 2017-07-20 Routine/Complete Inspection No Violations Noted Today's inspection, 7/20/17 is a routine inspection. The only well drilled and in production is the 3H. Site stable as is evidenced in all attached photos. Cellar is half full of water with no bubbles present. No violations noted.
2618205 Primary Facility 2017-07-24 Routine/Complete Inspection No Violations Noted Today's inspection, 7/20/17 is a routine inspection. The only well drilled and in production is the 3H. Site stable as is evidenced in all attached photos. Cellar is half full of water with no bubbles present. No violations noted.
2639233 Primary Facility 2017-09-21 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar had water above the (20x13) annulus with no bubbling noticed. There was a crew on site repairing the grating around the well. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2643311 Primary Facility 2017-09-06 Routine/Complete Inspection No Violations Noted Arrived on location at 2PM. One well on location. 175psi on tubing. 2 tanks, 1 treater. No violations noted.
2744632 Primary Facility 2018-06-22 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 5.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2837344 Primary Facility 2019-02-07 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 1' below grade in the cellar and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same production tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
