Requested to have cuttings on containment cleaned up from the area between the open top cuttings containers and mud tanks. No violations observed at time of inspection.

2542222 Primary Facility 2016-12-07 Routine/Complete Inspection No Violations Noted The Department was on site, to conduct a Routine Complete Inspection, in response to the submittal of a Restoration Report. The E&S Inspection Log indicated that Stephen P. Opeka conducted the most recent inspection on 12/1/16 and it was a Post-Precipitation Inspection. The receiving waters are an unnamed tributary to Bell Creek which is designated CWF.

A well tender was on site at the time of the Department's arrival. He appeared to be checking the various tanks on location. He explained that the 1H well has been shut in for a frac job on another site.

The Department contacted Mike Pirner (Cabot OG) while on site to inquire about the third party disturbance. He explained that it was pipeline work.

2552402 Primary Facility 2017-01-04 Site Restoration No Violations Noted On 1/4/2017, the Department conducted an inspection of the COG Davis G P1 Well Site located in Gibson Township, Susquehanna County. The inspection was conducted in response to COG’s submittal of a Notice of Termination form for this site. The Department finds that the site is permanently stabilized and that PCSM BMP’s have been emplaced. There are 2 wells located on this site, the 2H(115-20843) and the 3H(115-20844). Both wells appear to be in production. I recommend this Notice of Termination be approved.
2581115 Primary Facility 2017-04-10 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 8:50a.m.

The cellar was full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into the production tank, the (9x5) annulus vent pipe was open and venting into the production tank. There was 588psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:10a.m.

2687385 Primary Facility 2018-01-25 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of ice and unable to check for gas. The (20x13) annulus was below the ice level. The (13x9) and (9x5) annuli vent pipes were connected together and venting to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2751405 Primary Facility 2018-06-26 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:43. The weather conditions are mostly sunny skies and light winds with temperatures in the low to mid 70's. The was no activity and no Cabot personnel on-site during the inspection. The G Davis 2H and 3H wells are in production. I met with John Bankus, asset integrity specialist of Williams. He informed me that he was at the site to conduct an inspection of the Williams pipeline for cathodic protection and corrosion control. I used a Hanna HI 9831 Soil Test EC & Temp meter, a Hanna HI 9831 Groline Soil Test meter and an Oakton meter to conduct specific conductance (conductivity) screening in water in the sump for the G Davis Gas Production Units (GPU's) and containment unit for the production fluid storage tanks. Elevated conductivity readings were recorded in water in the gas production unit sump for the G Davis 3H well. See photos. I departed from the site at 13:41.
2785799 Primary Facility 2018-10-09 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 1' below grade in the cellar and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same production tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2849698 Primary Facility 2019-03-04 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. As the photo's show, the site is snow covered. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
