2447326 Primary Facility 2016-01-20 Incident- Response to Accident or Event No Violations Noted The Department was on site to conduct an inspection in response to two small spills at the Camp Comfort Well site. The wells associated with this site are:

115-21980, 115-21981, 115-21982, 115-21983, 115-21984, 115-21985, 115-21986, 115-20819, 115-20820, 115-20821, 115-20822, 115-20823, 115-20824.

The first spill was on 1/16/16 at 1600hrs. A power steering hose on a water truck on site broke, resulting in a 1/2 gallon release of power steering fluid to containment. It was vacuumed up immediately. Company man Billy Watts reported spill.

The second spill occurred on 1/20/2016 at approximately 0840hrs. A hydraulic hose on a man lift broke, releasing approximately 1 gallon of hydraulic fluid. The fluid and soil was immediately scooped up into a 5 gallon bucket. Golder and Associates removed soil. Company man Billy Watts reported spill.

There was adequate vegetation throughout the site, and there were no erosional issues apparent at this time.

2462701 Primary Facility 2016-03-04 Incident- Response to Accident or Event No Violations Noted The Department was on site to conduct an inspection in response to two small spills at the Camp Comfort Well Site (ESX11-115-0056) The wells associated with this site are:

115-21980, 115-21981, 115-21982, 115-21983, 115-21984, 115-21985, 115-21986, 115-20819, 115-20820, 115-20821, 115-20822, 115-20823, 115-20824.

Dell Gunn, company man for Talisman called in a small spill on March 4, 2016 to report a spill on the site. The spill occurred at around 0945 hrs. during fracking of the 10H well. When going to flush, the pressure spiked and the safety/pressure relief valve opened briefly, allowing approximately 5 to 10 gallons of an 80%fresh water - 20%flowback water to fall on containment. There was a vac truck on standby on site, and the spill was vac'd up immediately by Midosa. The Department inspected the site and all of the released water was cleaned up.

There was adequate vegetation throughout the site, and there were no erosional issues apparent at this time.

2475143 Primary Facility 2016-04-04 Follow-up Inspection No Violations Noted The Department was on site to conduct an inspection at the Camp Comfort Well Site (ESX11-115-0056) The wells associated with this site are:

115-21980, 115-21981, 115-21982, 115-21983, 115-21984, 115-21985, 115-21986, 115-20819, 115-20820, 115-20821, 115-20822, 115-20823, 115-20824.

The Department arrived on site at the Camp Comfort location at approximately 1400 hours. While on site for around 20 minutes, the Department did not make contact with any personnel. There was nobody in the foreman's trailer, nor were any personnel observed near the wells. The site was quiet and it is assumed there was flowback going on at this time. With the onset of rain, the Department vacated the site.

There was adequate vegetation throughout the site, and there were no erosional issues apparent at this time.

2501108 Primary Facility 2016-07-05 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection at the Camp Comfort Well Site (ESX11-115-0056). The wells associated with this site are:

115-21980, 115-21981, 115-21982, 115-21983, 115-21984, 115-21985, 115-21986, 115-20819, 115-20820, 115-20821, 115-20822, 115-20823, 115-20824.

   Jonathan Sassi and Frank Hoehle were on site to inspect the Camp Comfort well site.  The site had some ongoing monitoring and excavation work open due to a release earlier this year. (Photo 1)   When checked with a Hanna EC and Soil Probe HI98331, the areas around the excavations showed conductivity as follows:  at 41.83790 76.11297 there were readings of 1.7millisiemens (mS), and 1mS.  The soil in this area was slightly damp.  Downgradient of the previous spill area the soil was dry, with readings (in the vicinity of GPS location 41.83767  76.11309) as follows:  0.4mS, 0.46mS, 0.8mS and 0.64mS. 2549003 Primary Facility 2016-12-28 Incident- Response to Accident or Event No Violations Noted The Department was on site to conduct an inspection in response to a release (complaint number 324228) at the Camp Comfort Well Site (ESX11-115-0056). The wells associated with this site are:

115-21980, 115-21981, 115-21982, 115-21983, 115-21984, 115-21985, 115-21986, 115-20819, 115-20820, 115-20821, 115-20822, 115-20823, 115-20824.

Jack Bolyard (Talisman rep) notified Bob Bisignani (NERO-ER) of a spill which occurred at 10:00 PM on 12/25/2016. The spill was referred to the Scranton office through Alice Adams on 12/27/2016 at 11:30AM. 420 gallons (10 bbls.) of production fluid was released to containment on 12/25/2016 at around 6:30AM. The water in the containment froze and will be removed when the ice melts. Jon Sassi contacted Jack Bolyard from Talisman and Frank Hoehle inspected the site on 12/28/2016
