2062010 Primary Facility 2012-04-19 Incident- Response to Accident or Event Violation(s) Noted Arrived at site at 12:20 in response to release of (4) to (6) barrels (bbls) of synthetic oil- based mud (SOBM) that occurred between 6:00 and 6:30 AM on 4-19-12. Pattesron-Uti Rig # 538 is active drilling rig at the site. Current drilling depth is 5,352 feet. Mr. Webster reported release at 8:40 AM to the Department. Report indicated that (21) gallons of SOBM escaped containment & impacted surface of ground on Well Pad. The result of release was due to night shift derrick hand attempting to shut butterfly valve on tank labeled #4 and then opened the suction and discharge valves of tank labeled #5. He began circulating the tank and was relieved. The centrifugal pump was engaged and was pulling SOBM out of both tank #4 and tank #5 and circulating back into tank #5.

The (13x9) annulus vent pipe was open and venting into the production tank, the (9x5) annulus vent pipe was open and venting into the production tank. There was 522psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:45a.m.

The (13x9) and (9x5) annuli vent pipes were connected together and venting to the same tank. I took a walk around the location and there were no obvious concerns noticed. The well was in production. There was water 5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same production tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
