There were rig mats, plastic tanks and 55 gallon drums that were setting on a

The (13x9) vent pipe was open and venting into a small poly tank, the (9x5) vent pipe was open and venting into a small poly tank. There was 698psi on the production casing. Baker Hughes had frack equipment staged on site from the frack job at the Wright pad. I took a walk around the location and there were no obvious concerns noticed.

2616976 Primary Facility 2017-06-16 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection of the A Ellsworth Pad. I reviewed the E&S Inspection Log which stated that Anne Bonczek conducted a weekly inspection on 6/14/17. I contacted David Schoonover (DEP-OGI) when I noticed that the 4H GPU seemed to be making a louder than normal sound. He arrived on site during my inspection and looked at the unit.
2626892 Primary Facility 2017-08-16 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:30a.m.

The cellar was about a quarter full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 553psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:10a.m.

2737710 Primary Facility 2018-06-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water .5' below grade in the cellar and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2833900 Primary Facility 2019-01-31 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of snow and ice, I was unable to check for gas. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
