Operator did not determine TOC, and ran in hole with 1" tremie pipe to 145' and pumped 24 bbls with estimated returns of 6 bbls. Cement fell back to 45' below surface. Currently planning to top out again. Initial cement job completed 2 am this morning. Will complete 2nd top job later today and wait 8 hrs on cement. Operator will run bond log before drilling out. Operator to contact DEP when bond log complete. REportedly encountered 300 bbl/hr of freshwater at 298' while drilling surface hole. No shallow gas reported. Drilled to 455' and could only get casing down to 430' (kb).
2041370 Primary Facility 2012-01-08 Follow-up Inspection Pending Follow up on 13 3/8" cement job. As of 2/6/12 - had completed top job, and still had fall back to @ 45'. Since last inspection, completed 2nd final top job with another 48 sks Bondcem. Had reutnrs and cement stayed @ surface. No reported gas bubbling in cellar around well head. Met with Jim Jackson and Joe Ray of WPX on site. Bondlog reportedly showed good bond according to WPX. DEP requested a summary /timeline of 13 3/8" cement job activities and all associated logs.

I took a walk around the location and everything looked OK.
2508847 Primary Facility 2016-05-04 Routine/Complete Inspection No Violations Noted Inspection of the Kass well pad in conjunction with the Northeast Gathering Phase 3 Gathering System - Line 417 pipeline notice of termination inspection. There were no issues observed on the well pad at the time of the inspection.
2524714 Primary Facility 2016-09-29 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:10a.m., the cellar was full of gravel and water with no bubbling noticed at the perimeter of the cellar. The vent pipe was open and set up with a Taylor plug to check for pressure, there was 235psi on the production casing and 181psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 11:40a.m.

2615741 Primary Facility 2017-07-13 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:20p.m.

The cellar was full of gravel and there was water setting on the top with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 222psi on the production casing and 192psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 15:05p.m.

2637339 Primary Facility 2017-08-11 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection of the Kass Well Site. Bruce Mazur (Pumper) arrived on site during my inspection. Mr. Mazur explained that on 8/14/17 they will be drawing down the production tanks and at that same time they will remove the fluid from the containment. He explained that they are pumping freshwater downhole to help dilute the salt to increase production. The Kass 2H, 4H, and 6H are presently on site and each is connected to the tank.

I contacted Mike Deming (SWN) to notify him of the sheen found within the containment. He later returned my call. He explained that they will be vacuuming out the containment today. At this time I also advised him of the presence of several amphibians at different stages of their life cycle found within the containment. I recommended that SWN contact the PA Fish and Boat Commission for direction on how to handle the amphibian population living within the containment.

2719068 Primary Facility 2018-04-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2768440 Primary Facility 2018-08-28 Routine/Complete Inspection No Violations Noted I performed a Routine Complete Inspection of the Kass well pad with Casey Baldwin. The wells were properly labeled. There was no bubbling in the well cellars noted.

The production tank containment did not have any standing water.

The access road and pad surface were solid and stable.

The cut and fill slopes were well vegetated and stable. The vegetation was perennial and exceeded 70 percent.

There were no erosional issues noted at the time of inspection.

2804894 Primary Facility 2018-11-19 Routine/Complete Inspection No Violations Noted I peformed a brief RTNC inspection of the Kass well site. I had inspected this site in the recent past so a perfunctory inspection was performed as I was already on site. I was also on site to perform a FUI for the Kass to Snake Creek slide area. The area was recently blanketed with about 8" of snow. It was 35 degrees and raining slightly at the time of inspection.
    The access road was solid and had a cattle guard style entrance to knock mud off the tires.  
    The cut and fill slopes were well vegetated, visible through the heavy snow.  There was not any evidence of any slides or stability issues on any of the vegetated areas on site. 
    The wells were properly and clearly labeled.
    There were no erosional issues noted, and there were no violations observed.
2819855 Primary Facility 2018-12-27 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
